Objective: To analyze the clinical features of elderly patients with epithelial ovarian cancer treated in our hospital,and to explore the related factors affecting the efficacy of senile epithelial ovarian cancer.Methods: The patients who were treated by the Department of Gynecologic Oncology of the Affiliated Tumor Hospital of Guangxi Medical University from January 2007 to January 2017 were diagnosed as epithelial ovarian cancer by pathology.118 patients aged ? 60 years old were included in the study.Sexuality was randomly selected from 150 young patients aged <60 years.The clinical characteristics of surgical pathological staging,pathological type,tissue differentiation,initial cytoreductive surgery,chemotherapy course,etc.were analyzed,and the factors related to prognosis were analyzed by univariate and multivariate factors to explore the effects of elderly patients.Related factorsResults: 1.The proportion of patients with stage III-IV in the elderly group was higher than that in the young group,and the difference was statistically significant(P<0.05).The proportion of moderately differentiated patients in the elderly group was significantly lower than that in the younger group,and the difference was statistically significant(P<0.05).The proportion of poorly differentiated patients in the elderly group was significantly higher than that inthe young group,and the difference was statistically significant(P<0.05).The proportion of each pathological type in the two groups was not statistically significant(P>0.05).The satisfactory rate of tumor reduction in the elderly group was significantly lower than that in the young group,and the difference was statistically significant(P<0.05).In patients with lymph node dissection,the rate of pelvic and para-aortic lymph node metastasis in the elderly group was higher than that in the young group(P<0.05).The completion rate of chemotherapy in the elderly group was significantly lower than that in the young group,and the difference was statistically significant(P<0.05).2.Univariate analysis showed that surgical pathological staging,initial cytoreductive surgery,pelvic and para-aortic lymph node resection,chemotherapy course,chemotherapy regimen can affect the prognosis of elderly patients with epithelial ovarian cancer,and serum CA125 levels before treatment,whether Neoadjuvant chemotherapy,pathological type,and degree of tissue differentiation were not associated with prognosis.Multivariate analysis showed that surgical pathological staging,initial cytoreductive surgery was satisfactory,and chemotherapy regimen was an independent factor affecting the prognosis of elderly EOC patients.3.The analysis of postoperative chemotherapy in the elderly group showed that the median OS of paclitaxel plus carboplatin for three weeks or weekly treatment,non-paclitaxel plus carboplatin combined with chemotherapy and single drug carboplatin was 47.4 months,34.7 months,26.3,respectively.The median OS of paclitaxel plus carboplatin three-week regimen or weekly regimen was significantly longer than that of patients receiving non-paclitaxel plus carboplatin plus chemotherapy or single-agent carboplatin chemotherapy.The difference was statistically significant(P<0.05).Conclusion: 1.Elderly patients with ovarian cancer are mostly late andpoorly differentiated.The pelvic and abdominal aortic lymph node metastasis rate is high,it is difficult to perform satisfactory tumor cytoreductive surgery,and the number of postoperative chemotherapy courses is insufficient.2.Univariate analysis showed that the surgical pathological staging,the initial cytoreductive surgery,whether the pelvic and para-aortic lymph nodes were removed,the course of chemotherapy,and the chemotherapy regimen could affect the prognosis of elderly patients with epithelial ovarian cancer.Multivariate analysis showed that the surgical pathological staging,the initial cytoreductive surgery was satisfactory,and the chemotherapy course was an independent factor affecting the prognosis of elderly EOC patients.3.compared with paclitaxel and carboplatin three weeks or weekly treatment program,elderly patients with non-paclitaxel and carboplatin combined with chemotherapy,single drug carboplatin chemotherapy,the effect is poor,showing a significant shortening of OS.
