Analysis On The Construction Status Of Some Smoking Cessation Clinics In China In 2016

BackgroundTobacco dependence is a chronic addictive disease,and it is difficult for smokers to successfully quit themselves.Effective professional smoking cessation intervention can help smokers cope with physical and psychological addiction,and help them establish a social environment to support smoking cessation,so as to improve the success rate of smoking cessation.Cost-effective interventions to quit smoking and tobacco dependence include short advice,medication,counseling and a stop smoking hotline.Smoking cessation clinic is specialized on smokers to quit smoking intervention is an effective way and method,in terms of smokers to quit smoking cessation help and guidance,has irreplaceable function,in the diagnosis and treatment process of smokers is recommended to use drugs.At present,there are only bupropion hydrochloride sustained-release preparation and varnicolan tartaric acid in China,and none of them are included in the scope of medical insurance in China,severely restricting the use of anti-smoking drugs.Smoking cessation clinic in China started late,development is slow,but since 2014,the central subsidies to local projects across the country to carry out the smoking cessation outpatient construction work,greatly promoted the smoking cessation clinic the pace of progress in our country.This study is the first time to analyze the current situation of the smoking-cessation clinic construction in the project hospital,so as to provide more evidence for the construction of the smoking-cessation clinic in China in the future.ObjectiveTo understand the current situation of China's smoking cessation clinic construction from five aspects of institution construction,environment construction,process evaluation,result evaluation and development index evaluation.To understand the effect of smoking cessation intervention in China through the registration of smoking cessation clinics.MethodsBased on the central subsidies for local health literacy promotion action projects 2016 hospital outpatient quitting,collect the provincial experts in May and June 2017,the area under its jurisdiction smoking cessation clinic for external reviews get smoking cessation clinic evaluation questionnaire data,from the questionnaire of institutional construction,environment construction,process evaluation,outcome evaluation and development indicators evaluation analysis;The data of all the first visits and follow-up visits of smokers registered in the smoking cessation clinics of all project hospitals from July 2016 to June 2017 were collected.Using SPSS 19.0 statistical software for data for statistical analysis,mainly USES the method of statistical description and the service of smokers quit smoking outpatient construction condition is described,the measurement data with the mean,median,count data with frequency,composition ratio and rate,etc.,the comparative analysis with chi-square test.ResultsIn the 106 project hospitals in 2016,94 outpatient assessment questionnaires and 89 registration forms were collected.The average score was 64.60,the highest score was 93.00 and the lowest score was 24.00.There were 72 hospitals qualified for the evaluation of smoking cessation clinics,with a qualified rate of 76.6%.Big city hospital average score and pass rate is higher than small and medium-sized city hospital,the average score and pass rate of the third class hospitals is higher than other grades hospital,to create a hospital in the previous years average score and pass rate is higher than that year to create a hospital.There was no significant difference between executive department in the respiratory department and non-respiratory department.By comparison with smoking cessation clinic evaluation questionnaire scores five parts,according to the environment construction,organization construction points won the highest 89.4%and 85.9%,respectively,process evaluation,outcome evaluation serves generally were 71.6%and 64.5%respectively,the worst is the development of index evaluation serves only 32.3%.All items hospital outpatient basis quitting index score is better,development index score is bad,smoking cessation drugs at the biochemical detection equipment,long-term,to set up the referral system,medical staff special allowance funds,special funds smoking cessation clinic,quit smoking related research papers index serves is very low.From July 2016 to June 2017,the total number of first visits registered in the smoking cessation clinic was 5,679,with the average number of first visits per hospital being 66.The average age is 47.45 years old.The average smoking age is 20 years.The average daily smoking amount is 20 cigarettes.The number of patients enrolled in the monthly follow-up was 4150,and the average number of patients in each hospital was 51,with a follow-up rate of 73.1 percent.829 patients took drugs to quit smoking,accounting for 14.6 percent.122 cases were treated with amphetamine hydrochloride,498 cases were treated with vornicolan tartaric acid,106 cases were treated with traditional Chinese medicine,100 cases were treated with other drugs,and 3 cases were combined.After smoking cessation intervention in the outpatient clinic,the rate of smoking cessation was 25.1%at 7 days after the monthly follow-up.In the smoking cessation clinic with the use of bupropion hydrochloride,tartaric acid varnicolan,during one month of follow-up,the rate of quitting smoking at 7 days was 37.7%and 30.7%.ConclusionThe 2016 program covers a wide range of outpatient clinics for smoking cessation in hospitals,and the hospital types are diversified.The overall construction of smoking cessation clinics in all project hospitals is mostly at the middle level.The current construction status shows that the infrastructure construction is good and the capacity construction is poor.The overall construction is very different and uneven.It is suggested that different key points should be made for different types of hospitals to strengthen the construction of weak links.The average annual outpatient visits for smoking cessation clinics are low,so it is suggested to strengthen the hospital referral system and internal and external publicity to increase the number of outpatient visits.The effect of smoking cessation intervention in the outpatient department is obvious.The effect of drugs on smoking cessation is obvious.It is recommended to use drugs in the clinic.The proportion of drugs used in the outpatient service is low,so it is suggested to increase the publicity of drugs for smoking cessation,to include drugs for smoking cessation into the medical insurance,and to increase the awareness rate and utilization rate of drugs for smoking cessation.
