| Chronic atrophic gastritis(CAG)is a common chronic disease in digestive department.With the development of economy and the change of people’s living and eating habits,the incidence and detection rate of CAG in young people are increasing year by year,which seriously affects their work,study and quality of life.With the growth of age,the risk of long-term canceration deterioration is also greater.Modern medicine has no specific treatment for this disease,but traditional Chinese medicine has a profound understanding of the etiology and pathogenesis of CAG.By using the system of"syndrome differentiation and treatment"of traditional Chinese medicine,we have made relatively positive achievements in the treatment of CAG.Clinical practice has proved that taking traditional Chinese medicine can significantly improve the clinical symptoms of the disease,and has a positive effect on atrophy and intestinalization.However,there is no specific research on CAG for young people.To sum up,this paper will make a further study on the distribution characteristics of atrophic gastritis syndrome types in young people and the therapeutic effect of TCM differentiation of symptoms and signs.Part one:Theoretical StudyThrough summarizing the understanding and opinions of traditional medicine and moderm Chinese medicine on CAG and combining with clinical observation,it is concluded that there are some differences between young CAG and old CAG in etiology,pathogenesis,clinical symptoms,physiological and pathological characteristics.Traditional Chinese medicine plays an irreplaceable role in the treatment of young CAG.Part two:Clinical ObservationObjective:Analyze and summarize the distribution characteristics of youth CAG;Using the method of"self-control",the effect of traditional Chinese medicine on CAG in young people was discussed from the changes before and after patients took traditional Chinese medicine decoction,and an effective treatment method was summarized.Method:General information,four diagnostic data,TCM symptoms,endoscopy and histopathological results of young CAG patients in digestive outpatient and ward of our hospital were collected.The data of 112 young CAG patients were analyzed and processed,and the distribution regularity of syndrome types was summarized.The curative effect of 69 young CAG patients who insisted on taking traditional Chinese medicine decoction was observed.The treatment period was 3 months.The patients were reexamined after 6 months of treatment.The symptoms,signs and histopathological results of the patients before and after 6 months of treatment were recorded.IBM SPSS Statisti was used.CS 21.0 software carries on the statistics,the analysis and the processing to the data,and carries on the clinical curative effect appraisal.Results:In the treatment of young CAG with TCM,the total score of TCM syndromes decreased significantly after 6 months of treatment compared with the baseline before treatment(P<0.01),and the baseline scores of TCM syndromes also decreased after 6 months of treatment compared with that before treatment(P<0.01 or P<0.05).In terms of pathological score,the atrophy and Intestinalization of the greater curvature of the antrum were decreased afiter 6 months of treatment compared with those before treatment(P<0.01 or P<0.05),and the atrophy and Intestinalization of the lesser curvature of the antrum were also decreased after 6 months of treatment compared with those before treatment(P<0.01).The main symptoms of TCM patients were cured in 4 cases(5.8%),markedly effective in 19 cases(27.5%),effective in 40 cases(58%),ineffective in 6 cases(8.7%)and effective in 63 cases(91.3%).As for the pathological effect,after 6 months of treatment,38 cases of gastric atrophy were cured,accounting for 55.1%;14 cases were effective,accounting for 20.3%;17 cases were ineffective,accounting for 24.6%,and the total effective rate was 75.4%;15 cases were cured,accounting for 27.3%;1 case was markedly effective,accounting for 1.8%;8 cases were effective,accounting for 14.5%;31 cases were ineffective,accounting for 56.4%,and the total effective rate was 43.6%.Conclusion:In young patients with atrophic gastritis,spleen-stomach weakness and liver-stomach disharmony syndrome are the most common,but the overall proportion of excess syndrome is higher than deficiency syndrome,mainly empirical.The clinical effect of TCM on young patients with atrophic gastritis is remarkable.In terms of TCM symptoms,TCM decoction is mild,and has advantages in individualization,integrity and persistence,which greatly alleviates the suffering of patients and greatly improves their quality of life.In the aspect of histopathology,TCM plays a certain role in reducing the degree of atrophy of gastric mucosa and intestinal metaplasia,and has a positive significance in slowing the progression of atrophic gastritis to early gastric cancer. |
