Preliminary Clinical Study On The Relationship Between Phlegm-dampness Syndrome,Blood Stasis Syndrome And Sleep Disorders In Stage Ⅲ-Ⅳ Diabetic Kidney Disease

Objective:Through a cross-sectional survey of phlegm-dampness syndrome,blood stasis syndrome and sleep quality in diabetic kidney disease(DKD);to explore the correlation between blood stasis syndrome,phlegm dampness syndrome and sleep disorders in DKD;to analyze the correlation between the phlegm-dampness syndrome,blood stasis syndrome of DKD and clinical biochemical indicators.It is confirmed that the phlegm and blood stasis syndrome will affect the sleep quality of DKD patients,and provide a theoretical basis and preliminary basis for the prevention and treatment of DKD patients’ sleep problems.Methods:This study used a questionnaire survey methodology.A total of 100 patients with stage III-IV DKD who are undialyzed,inpatient and outpatient department in nephrology,the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine were randomly selected for sleep quality survey,and their blood stasis and phlegm dampness syndrome scores were evaluated.The Blood Stasis Scale refers to the“Diagnostic Criteria for Practical Blood Stasis Syndrome”led by Academician Keji Chen.The phlegm dampness stasis scale refers to the“Clinical Diagnostic Criteria for Coronary Heart Disease of Dampness Syndrome”led by Jingqing Hu,and the PSQI scale is used in the sleep quality survey scale.To collect general information,sleep quality,blood stasis and phlegm dampness syndrome score,clinical biochemical indicators and diabetes complications of patients with DKD,and use statistical methods to analyze the frequency distribution of TCM symptoms of blood stasis syndrome and phlegm-dampness syndrome in DKD;to explore the correlation between phlegm dampness syndrome,blood stasis syndrome and sleep disorders in DKD patients,mainly from six aspects:sleep quality,sleep time,sleep efficiency,sleep influencing factors,daily dysfunction,and hypnotic drugs;and analyze the correlation between phlegm dampness syndrome,blood stasis syndrome and clinical biochemical indicators in patients with DKD;and explore the correlation between phlegm dampness syndrome,blood stasis syndrome and DM complications in patients with DKD.Results:1.In terms of sleep disorders,there is a positive correlation between blood stasis syndrome score and sleep time,sleep efficiency,daily dysfunction and PSQI total score.No linear correlation is found between blood stasis syndrome score and self-assessment of sleep quality,sleep AIDS and sleep influencing factors.There is a positive correlation between the phlegm-dampness syndrome score and daily dysfunction,sleep aid and PSQI total score.No linear correlation is found between phlegm-dampness syndrome score and self-assessment of sleep quality,sleep time,sleep efficiency and sleep influencing factors.2.In terms of coagulation function,there is a positive correlation between blood stasis syndrome score and D-dimer,no linear correlation is found between blood stasis syndrome score and PLT,PT,INR,APTT and FIB.In terms of biochemical indicators,there is a positive correlation between blood stasis syndrome score and urea nitrogen,serum creatinine and cystatin C,and a negative correlation between blood stasis syndrome score and eGFR、HbA1C,no linear correlation is found between blood stasis syndrome score and fasting blood glucose、serum albumin.In terms of urinalysis,there is a positive correlation between blood stasis syndrome score and ACR,and no linear correlation is found between blood stasis syndrome score and urinary leukocytes,urinary red blood cells,total 24-hour urine protein.No linear correlation is found between phlegm-dampness syndrome score and biochemical indicators,Blood lipid,urine protein.3.There is a positive correlation between blood stasis syndrome score and DM complication score.No linear correlation is found between the phlegm-dampness syndrome score and the DM complication score.Conclusion:1.Blood stasis syndrome and phlegm dampness syndrome scores are related to sleep disorders in DKD patients.Severe blood stasis syndrome may lead to prolonged sleep in DKD patients,low sleep efficiency,daily dysfunction,etc.Taking sleeping pills may aggravate dampness syndrome,or the two interact with each other.2.Blood stasis syndrome will aggravate the renal function failure of patients with DKD and increase proteinuria.3.Blood stasis syndrome will aggravate the occurrence of complications of diabetes.
