OBJECTIVE: To investigate the effects of different gestational weeks on termination of pregnancy in early-onset severe preeclampsia patients and the choice of delivery methods.Methods: The clinical data of early-onset severe pre-eclampsia patients admitted to the First Affiliated Hospital of Hebei North University from January 2017 to December 2017 were retrospectively analyzed.100 patients were included and divided into 3 groups according to their gestational weeks of termination of pregnancy.In group A,28 weeks ? gestational age < 32 weeks(26 cases),group B 32 weeks ? gestational weeks< 34 weeks(32 cases),group C gestational weeks ? 34 weeks(42 cases).Record the age,pregnancy,blood pressure at admission,general information,admission treatment and outcomes such as expected treatment time,maternal complications,mode of delivery,neonatal birth weight,perinatal concurrency Clinical data such as symptoms.Statistical methods were used to analyze the duration of treatment,the occurrence of complications,the mode of delivery and the outcome of perinatal infants in each group.The test level was ?=0.05,and the difference was statistically significant at P<0.05.Results: There was no significant difference in the age,pregnancy and systolic blood pressure and diastolic blood pressure between the three groups(P>0.05).2.The expected treatment time of the three groups of patients was compared with each other.The average expectation time of the C group was the longest,and the B group was the second,the difference was statistically significant(P<0.05).3.Comparison of the mode of delivery in the three groups.The cesarean section was the main mode of delivery in each group.The statistical analysis of the cesarean section rate between the three groups was not statistically significant(P>0.05).4.The complications of pregnant women in the three groups were compared with each other,the difference was not statistically significant(P<0.05),and there was no significant difference in the incidence of maternal complications between the two groups(P<0.05).Hypoalbuminemia was the most common complication in each group.Placental abruption and liver and kidney dysfunction were second.Among the cases of hypoalbuminemia,statistical analysis was performed between the three groups.The difference was statistically significant.Significance,the remaining complications,the difference between the three groups was not statistically significant.5.Statistical analysis of three groups of perinatal complications found that the incidence of neonatal asphyxia,neonatal death and small for gestational age in each group gradually decreased with the increase of gestational age,and the difference was statistically significant.The significance of learning(P< 0.05),there was no significant difference between the three groups in the fetal distress and stillbirth(P>0.05).There was a significant difference in neonatal death between group A and group B(P<0.05).There was a difference between group A and group C in neonatal asphyxia,neonatal death and small for gestational age.Statistical significance(P<0.05),the difference between group B and group C was not statistically significant(P>0.05).Conclusion: 1.With the prolongation of gestational age,the incidence of maternal and child complications in patients with early onset severe preeclampsia is decreasing,and the incidence of neonatal asphyxia,neonatal death and small for gestational age is significantly reduced.Severe preeclampsia is extended to 34 weeks and the pregnancy is terminated,and a satisfactory maternal and child outcome can be obtained.2.It is necessary and feasible to carry out the expectation treatment under the premise of ensuring the safety of the mother.Under the premise of close monitoring,the gestational age can be extended as much as possible,and the ideal maternal fetal outcome can be obtained.3.The method of termination of pregnancy in patients with early-onset severe pre-eclampsia is mainly cesarean section.
