Objective:To discuss the terminal gestational age and the delivery type for maternal and perinatal prognosis with severe preeclampsia.Methods:269 patients from 28 weeks to over 28 weeks with severe preeclampsia were retrospectively analyzed and summarized by the gestational age and delivery type,comparing the outcome of different gestational weeks and type of delivery of the pregnancy to pregnant women and newborns. Results: Different terminal gestational age of pregnant women had significant differences in blood pressure,onset gestational age,terminal gestational age,creatinine,albumin,the qualitative of urinary protein,the quantitative of 24 h urinary protein,D-dimer,alanine aminotransferase,placental abruption (P<0.05). Different gestational age of newborns had significant differences in stillbirth,perinatal death,Weigh,asphyxia,,fetal growth restriction,neonatal hyaline membrane degeneration,infection,intracranial hemorrhage areas (P<0.05). There is a significant difference in stillbirth with different delivery type (P<0.05),other respects have not for newborns (P>0.05).The result of multiple variables pair-logistic regression analysis motherhood times,systolic blood pressure,placental abruption exhibit statistical significance on perinatal death.Conclusion: The onset gestational age earlier,the disease stronger,the clinical complications more, the terminal gestational age earlier.34 weeks gestation is time point that improve newborns prognosis.Cesarean section is not a very effective means to improve the quality of newborns' lives.The choice of delivery type should be based on maternal and fetal specific circumstances.Motherhood times,systolic blood pressure,placental abruption are risk factors for perinatal death.
