Objective:Platelet distribution width(PDW)is an indicator of platelet morphology in blood routine.The larger the platelet distribution width,the greater the disparity in platelet volume and the higher the degree of platelet activation.In this article,through analysis of acute coronary syndrome(ACS)in patients with platelet distribution width value and left ventricular ejection fraction(LVEF)value,to explore the platelet distribution width and acute coronary syndrome and the correlation of left ventricular ejection fraction.Methods:Patients who were admitted to the emergency department of the affiliated hospital of chengde medical college from January 2017 to December 2017 with chest pain with blood sampling test in the emergency department department and did coronary angiography to determine the lesion site by an interventional cardiologist were collected.313 cases were included in the ACS group.45 patients with normal coronary angiography results were included in the control group.Clinical data of the patients(gender,age,smoking history,drinking history,history of hypertension,history of diabetes,TCH,TG,ldl-c,hdl-c,platelet distribution width,left ventricular ejection fraction)were recorded respectively.PDW and LVEF of ACS group and control group were compared.The ACS group was divided into Acute myocardial infarction(AMI)group and unstable angina(UA)group.The levels of PDW and LVEF in the two groups were analyzed and compared with those in the control group.According to the results of coronary angiography,patients were scored according to Gensini Score,and the total Score was divided into three grades: no more than 30 group,more than 31 and less than60 group,more than 60 group.PDW and LVEF were compared between the two groups,so as to explore the relationship between PDW and LVEF and coronary artery disease.Correlation analysis was performed for PDW value,LVEF and Gensini integral in the abnormal group of coronary angiography results,so as to obtain the correlation between PDW,LVEF and Gensini.Patients in the ACS group were divided into two groups according to left ventricular ejection fraction: LVEF less than45%,more than 45%,according to the group PDW for ROC curve analysis,so as to analyze the predictive value of PDW for left ventricular ejection fraction.SPSS19.0 was used for statistical analysis.Results:1.The PDW value(11.46±1.47)and LVEF(64.42±6.35)of the control group were significantly different from those of the ACS group(12.02±1.68)and LVEF(56.40±8.77)(P<0.05).2.The PDW value(12.13±1.73)and LVEF(55.23±8.67)in AMI group were significantly different from those in UA group(11.67±1.48)and LVEF(60.33±7.98)(P<0.05).3.PDW value increased gradually with the increase of Gensini integral.Except that the difference between the group of<30 and the control group was not statistically significant,the difference between the other groups and the control group and the groups was statistically significant(P<0.05).LVEF decreased with the increase of Gensini integral,and the differences between groups were statistically significant(P<0.05).4.PDW value was positively correlated with Gensini integral(R=0.208,P<0.05).LVEF is negatively correlated with the Gensini integral(R=0.506,P<0.05).5.LVEF<45% was predicted by PDW value was used as ROC curve,and the area under AUC curveThe area under the AUC curve(0.538,95%CI: 0.448~0.629,P=0.046).Conclusion:1.The Platelet distribution width level in the ACS group was higher than that in the normal control group,and the Platelet distribution width level in the AMI group was higher than that in the UA group.Platelet distribution width may play a role in the diagnosis of ACS;2.Platelet distribution width was positively correlated with Gensini integral,while left ventricular ejection fraction was negatively correlated with Gensini integral.Both Platelet distribution width and left ventricular ejection fraction may be related to the degree of coronary artery lesions.may be a predictor of left ventricular dysfunctio;3.Platelet distribution width may play a role in predicting decreased left ventricular fraction in ACS patients.
