Objectives To analyze the clinical effect of different dose prophylaxis of recombinant human coagulation factor ? for different frequency prophylaxis in children with hemophilia A.Methods Collect forty-eight male cases of severe type of hemophilia A patients who came to the pediatric outpatient department of our hospital,and these children aged 4-14 years old all accept preventive treatment from November 2017 to October 2018 in Tangshan and surrounding areas.After obtaining the consent of the patient or his/her family members,the child's medical history should be recorded in detail(obtaining the basic situation of the children in the form of questionnaire),Divided it into three groups:A,B and C,group A received moderate doses of prophylactic therapy(coagulation factor ? 20u/(kg.times)),twice a week;Group B received medium-dose prophylaxis(coagulation factor ? 15u/(kg.times)),3 times a week,Group C was the low-dose prophylactic treatment group(10 U/kg each time,2 times/week).All three groups received the first score of joint structure and function and quality of life(refer to SF-36 quality of life score)after enrollment,for a period of half year,observe the change of the most serious single Target joint score Using HEAD-US-C Ultrasonic score sheet which refers to Melchiorre Scoring system,HJHS score for joint and quality of life score as well as the bleeding frequency and degree.Assess the joint and the quality of life for a second time after half a year.Then conduct the third assessment after one year to study the later effects and progression of joint and life quality of different dose group at different frequencies and compare their curative effect.Results 1 There was a difference in the progression of joint damage and bleeding frequency between group A and group B within 1 year(P<0.05),but the opposite to the SF-36 Quality of life(P>0.05).There was no difference in the HJHS score of joint function between B and C(P>0.05),but was significance to the other aspects between group B and group C(P<0.05).There was an obvious difference in the progression of joint damage and bleeding frequency as well as the Quality of life between group A and group C(P<0.05).Conclusions 1 There were significant differences in the improvement of joint injury,quality of life score,bleeding frequency and other aspects in children with severe hemophilia a with different doses and different frequencies.The medium dose for high frequency group had better clinical effect.2 There was no significant difference in joint function between the low-frequency medium group and the low-dose group,but has different in other aspects.If only consider the clinical effect,advocate high frequency prevention treatment of medium dose,Otherwise advocate low frequency prevention treatment of medium dose for overall consideration.Figure 3,Table 8,Reference 77...
