Objectives Platelet derived growth factor-BB?PDGF-BB?is a kind of peptide regulatory factor that can stimulate the growth of tissue cells[1]and has the function of inducing the proliferation and migration of vascular endothelial cells and lumen formation[2-4].PDGF-BB can also maintain the integrity of vascular structure by recruiting pericytes[5-7].Diabetic retinopathy?diabetic retinopathy,DR?is a common blinding retinal complications of diabetes,its blindness caused by retinal microcirculation are the main mechanism of the formation of retinal capillary without perfusion area,cause retinal ischemia,to produce new blood vessels in eyes,leading to vitreous body hematocele tractional retinal detachment,and neovascular glaucoma,etc.Current studies have recognized that the development of DR is related to the excessive synthesis of various growth factors in the retina and the excessive inhibition of inhibitory factors,leading to the formation of new blood vessels and cell proliferation.In this study,PDGF-BB levels in peripheral blood and aqueous humor of DR patients and control patients were measured and analyzed,and the correlation between PDGF-BB level in peripheral blood and aqueous humor and DR was also discussed,providing experimental basis for the development of type 2 diabetes DR.Methods All the cases were from patients who underwent cataract surgery in the emergency general hospital.According to the clinical staging criteria of DR in the 2014clinical guidelines for the diagnosis and treatment of diabetic retinopathy in China[8],the cases were divided into four groups:control group and NDR group?no diabetic retinopathy?;NPDR group?non-proliferative diabetic retinopathy?;PDR group?proliferative diabetic retinopathy?.Control group:There were no other systemic diseases except hypertension and diabetes mellitus,and there were 75 cases of cataract surgery patients without other eye diseases except cataract.NDR group:25 patients with type 2diabetes without diabetic retinopathy underwent cataract surgery.NPDR group:25 patients with nonproliferative diabetic retinopathy underwent cataract surgery.PDR group:25patients with proliferative diabetic retinopathy underwent cataract surgery.Serum and aqueous humor of all patients were collected,and PDGF-BB mass concentration was determined by ELISA.All data were analyzed with IBM SPSS statistics 24 statistical software,and recorded in the form of meanąstandard deviation.Univariate analysis of variance?ANOVA?was used to compare the differences between groups,and LSD-t test with the minimum significant difference was used for multiple comparisons.Pearson correlation analysis was used for each group.Results 1 The concentration of PDGF-BB in serum was significantly different among the groups?P<0.01?.LSD-t test showed that P>0.05 between the control group and the NDR group,P>0.05 between the NPDR group and the PDR group,and P<0.01 between the other groups.Mean PDGF-BB concentration in serum:NPDR group and PDR group>control group and NDR group.2 The concentration of PDGF-BB in aqueous humor was significantly different among the groups?P<0.01?.LSD-t test showed that P>0.05 between the control group and the NDR group,and the difference was significant between the other groups?P<0.01?.Mean concentration of PDGF-BB in aqueous humor:PDR group>NPDR group>control group and NDR group.3 There was a significant correlation between PDGF-BB concentration in serum and macular thickness in the PDR group?P<0.05?.There was no significant correlation between PDGF-BB concentration in serum and macular thickness in other groups?P>0.05?.4 There was a significant correlation between the concentration of PDGF-BB in aqueous humor and the thickness of macula in the NPDR group and the NDR group?P<0.05?,while there was no significant correlation between the concentration of PDGF-BB in aqueous humor and the thickness of macula in the control group?P>0.05?.5 Except NPDR group?P<0.05?,there was no significant correlation between serum aqueous humor PDGF-BB concentrations in each group?P>0.05?.6 Multiple regression analysis showed that serum PDGF-BB concentration was correlated with the course of fasting blood glucose?FPS?,triglyceride?TG?and diabetes?P<0.05?.The concentration of PDGF-BB in aqueous humor was correlated with the course of diabetes mellitus?P<0.05?.Conclusions 1 Course of disease:The level of PDGF-BB in aqueous humor was only correlated with the course of diabetes mellitus.There was a correlation between the level of PDGF-BB in serum and fasting blood glucose,triglycerides and the course of diabetes mellitus.And there was no significant correlation between the concentration of PDGF-BB in serum and the concentration of PDGF-BB in aqueous humor in all the groups except the NPDR group.This suggests that the concentration of PDGF-BB in serum is more influenced by the whole body,and it may be more accurate to evaluate the development of diabetes with the concentration of PDGF-BB in aqueous humor.Diabetic retinopathy:The mean concentration of PDGF-BB in serum in the NPDR group and the PDR groups>the mean concentration of PDGF-BB in serum in the NDR groups and the control group only indicated that PDGF-BB concentration in serum in diabetic retinopathy patients were higher than those without diabetic retinopathy.2 Mean concentration of PDGF-BB in aqueous humor:PDR group>NPDR group>control group and NDR group.This indicated that the concentration of PDGF-BB in aqueous humor began to increase with the occurrence of diabetic retinopathy and further increased with the progression of diabetic retinopathy.However,there was a correlation between the concentration of PDGF-BB both in serum and aqueous humor in the NPDR phase,suggesting that the concentration of PDGF-BB in aqueous humor might be more meaningful to evaluate the development of DR in the PDR phase.3 Macular edema:the concentration of PDGF-BB in serum was only correlated with the thickness of macula in PDR group,and the concentration of PDGF-BB in aqueous humor was correlated with the thickness of macula in BDR group and PDR group.However,there was a correlation between the concentration of PDGF-BB in serum and the concentration of PDGF-BB in aqueous humor in the NPDR group,suggesting that concentration of PDGF-BB in aqueous humor was not accurate in response to DR in NPDR phase.4.The level of PDGF-BB in aqueous humor increased with the aggravation of DR lesion,which was correlated with macular edema and significantly correlated with the course of diabetes mellitus.Since the level of PDGF-BB in aqueous humor is affected by the concentration of PDGF-BB in serum in the NPDR phase,it may be considered as a biomarker in the PDR phase.Figure12;Table7;Reference 133.
