Objective: To explore the relationship between the level of handgrip strength(HGS)and nutritional status in patients with maintenance hemodialysis(MHD).Methods: A cross-sectional survey was conducted to collect the clinical data and nutritional parameters of 124 patients receiving regular dialysis treatment in the dialysis center of The People's Hospital of Hechi from March to September 2018.Relevant clinical data of patients were collected,handgrip strength level was measured by dynamometer,blood biochemical indicators,anthropometric indicators and modified quantitative subjective global assessment(MQSGA)was used to assess the nutritional status of patients.The t-test or chi-square test was used to compare the clinical data of male and female patients,the analysis of variance or chi-square test was used to compare the patients in the different handgrip strength groups,Spearman correlation analysis and multiple linear regression were used to analyze the correlation between handgrip strength and nutritional status in MHD patients.Results: The average handgrip strength of 124 patients was(22.95 ± 7.81)kg,and the median duration of dialysis is 26(12,45)months,including 80 males(64.5%)and 44 females(35.5%).There were significant differences in,height,MQSGA score,mid-arm circumference(MAC),mid-arm muscle circumference(MAMC),serum albumin(ALB),serum creatinine(Scr),fast plasma glucose(FPG),hemoglobin(HGB),and Urea clearance index(Kt/V)between the four groups(P < 0.05).Spearsman correlation analysis showed that height,weight,HGB,ALB,Scr,Uric acid(UA),MAC,and MAMC were positively correlated with handgrip strength(P < 0.05),while Kt/V,FPG,MQSGA scores and red cell volumedistribution width(RDW)were negatively correlated with handgrip strength(P < 0.05).Multiple linear regression analysis showed that after adjusting the variables such as gender,age,ethnicity,height,weight,BMI and so on,the MQSGA score increased by one point,the handgrip strength level of male patients may decreased by 0.58 kg,while the handgrip strength of female patients may decreased by 0.52 kg,and ALB,FPG and Scr are influencing factors of handgrip strength in male MHD patients.For every 1 g/L increased in ALB,the handgrip strength level of male patients may increased by 0.66 kg;for every 1 umol/L increased in Scr,the handgrip strength level of male patients may increased by 0.01 kg;and for every 1 mmol/L increased in FPG,the handgrip strength level of male patients may decreased by 0.69 kgConclusion: Handgrip strength level was positively correlated with nutritional status in MHD patients.With the decline of nutritional status,the handgrip strength level of MHD patients gradually decreased,Lower handgrip strength indicated a lower nutritional status of patients.Lower handgrip strength is a potential marker of malnutrition in MHD patients.The measurement of handgrip strength are worthy of further application in hemodialysis patients.
