Objective: This study aims to analyze and summarize the relationship between H.pylori infection and various TCM syndromes by observing the signs of gastric ulcer under the guidance of the tutor and on the basis of previous studies,so as to provide an objective basis for TCM syndrome differentiation and classification and make clinical diagnosis more rapid and accurate.For clinicians in the treatment of this disease,give full play to the characteristics of traditional Chinese medicine,a more comprehensive diagnosis.Methods:A total of 160 cases of gastric ulcer patients who met the research criteria were conducted syndrome differentiation and classification according to their syndromes in the Chinese medicine physicians at the level of deputy director or above,and the correlation between each type and the signs of peptic ulcer under gastroscopy was analyzed,and the correlation between each type and H.pylori infection was also analyzed.Results:(1)This study of 160 cases of gastric ulcer activity(A)and heal period in patients with stage(H),the syndrome of dampness-heat of spleen and stomach have the largest proportion,the syndrome of static blood in stomach collaterals have the smallest proportion,according to theproportion is in turn: the syndrome of dampness-heat of spleen and stomach(25.6%)> the syndrome of heat stagnation in liver and stomach(21.9%)>the syndrome of incoordination between liver and stomach(18.8%)>the syndrome of deficient cold of spleen and stomach(15.0%)>the syndrome of deficiency of atomac-yin(10.6%)> the syndrome of static blood in stomach collaterals(8.1%).(2)There were 101 cases of hyperemia and edema around ulcer under gastroscope,and 59 cases without hyperemia and edema.There were 79 cases with gastric mucosal erosion and 81 cases without gastric mucosal erosion.The results showed that the distribution differences of each syndrome type in the hyperemia and edema around the ulcer and erosions were significant(P<0.01),that is,the traditional Chinese medicine syndrome types were correlated in the hyperemia and edema around the ulcer and gastric mucosal erosions,and the syndrome of dampness-heat of spleen and stomach and the syndrome of heat stagnation in liver and stomach were more common than the other four types.There were 47 cases with yellow lichen yellowand113 cases with white lichen yellow under gastroscope;There were 36 cases with bile reflux and 124 cases without bile reflux.;The results showed that there was no statistically significant difference in the distribution of different syndromes in the lichen color of ulcer and the presence or absence of bile reflux(P>0.05).(3)There were 96 cases in the active stage of ulcer and 64 cases in the healing stage.The distribution comparison of each syndrome type in the active stage and the healing stage of gastric ulcer under gastroscope showed that the distribution difference of each syndrome type in the active stage and the healing stage of gastric ulcer was statistically significant(P<0.05).Among them,in active stage the syndrome of dampness-heat of spleen and stomach and the syndrome of heat stagnation in liver and stomach were more common than other four types,in healing stage the syndrome of deficient cold of spleen and stomach?the syndrome of dampness-heat of spleen and stomach and the syndrome of heat stagnation in liver and stomach were more common than other three types.(4)Among the samples selected in this study,there were 117 cases of positive in H.pylori,including 19 cases with bile reflux and 98 cases without bile reflux.43 cases of H.pylori were negative,including 17 cases with bile reflux and 26 cases without bile reflux.The data of the two groups were compared,and the results showed that there was a significant difference between H.pylori infection and the presence or absence of bile reflux(P<0.01).Non-bile reflux was more common in H.pylori positivity,indicating a negative correlation between H.pylori and bile reflux to some extent.(5)There were 117 cases of H.pylori positive and 43 cases of H.pylori negative gastric ulcer under gastroscopy.The distribution comparison of each syndrome type in H.pylori infection showed that the difference in the distribution of H.pylori infection among all the syndromes was significant(P<0.01).In H.pylori positive patients,the syndrome of dampness-heat of spleen and stomach and the syndrome of heat stagnation in liver and stomach were more common.Conclusion:(1)The total pathogenesis of gastric ulcer is the deficiency of the inner and excess of the excess,the basic pathogenesis of activity period is the dampness-heat of spleen and stomach ?the heat stagnation in liver and stomach,mainly were the excess of pathogenic qi;The basic pathogenesis of the healing period is the intermingled deficiency and excess that is,the deficiency of the origin of the weak spleen and stomach is exposed on the basis of the pathogenic factors in the active period.(2)The gastric ulcer under gastroscope hyperemia edema,mucosal erosion,H.pylori infection positive and active period in the TCM syndrome type distribution exist certain differences,the syndrome of dampness-heat of spleen and stomach and the syndrome of heat stagnation in liver and stomach were relatively common,that can be indirectly concluded that H.pylori infection and ulcer activity show a certain correlation,the main cause of congestion edema,mucosal erosion and ulcer,that is in keeping with modern medical research.(3)H.pylori has a significant relationship with the syndrome of dampness-heat of spleen and stomach and the syndrome of heat stagnation in liver and stomach.Therefore,patients with gastric ulcer syndrome differentiation should actively carry out the examination of H.pylori infection.(4)It is speculated that H.pylori belongs to the category of foreign "heat pathogen",which can be further confirmed by expanding sample study.
