Objective:To observe clinical effect of posterior scleral reinforcement surgery for the treatment of pathologic myopia.Methods:Retrospective analysis of the 20cases(38eyes)after posterior scleral reinforcement.The changes of best corrected visual acuity,axail length,OCT,electrophysiology and complication were observed before and after surgery.Results:The axial lengths of 1 month,3 months,6 months and 1year after surgery were(30.54±1.97)mm,(28.61±1.93)mm,(28.99±2.37)mm,(29.23±2.37)mm,(29.37±2.25)mm,(29.49±2.31)mm respectively,which was significantly shorter than preoperative,and the difference was statistically significant(P <0.05).Six months after surgery,14 eyes(73.68%)were essentially reattached,4 eyes(21.06%)were partially reattached and 1eye(5.26%)was not reattached.1 year after surgery,16eyes(84.21%)were essentially reattached,and 3 eyes(15.79%)were partially reattachedThe BCVA of the patients at preoperative and postoperative,were0.66±0.49 and 0.45±0.38,it was improved and the difference was statistically significant(P<0.05).Preoperative a amplitude was(-35.29±15.96)?V and b amplitude(74.54±35.20)?V,postoperative a amplitude was(-50.31±30.36)?V and b amplitude(101.63±32.55)?V,and the difference was statistically significant(P<0.05).The preoperative intraocular pressure was(14.34±3.07)mmHg,and the postoperative 1day,1 month,3month,6 month and 1year were(13.45±2.07)mmHg,(13.56±2.70)mmHg,(13.80±2.36)mmHg,(14.19±1.96)mmHg,(13.9±1.72)mmHg.The difference was not statistically significant(P>0.05).After posterior scleral contraction surgery,no retinal detachment,vitreous hemorrhage and other serious complications occurred.Conclusion:Posterior scleral contraction surgery can shorten the axail length and improve visual function,without malignant postoperative complications.It is safe and effective in the treatment of pathologic myopia.But it still takes a long time,a large sample of clinical observation and a controlled trial to demonstrate its long-term clinical effects.
