Objective:The purpose of this study was to investigate the correlation between platelet activation level and severity of coronary artery stenosis in patients with coronary heart disease by observing the degree of platelet activation and its difference after the onset of different types of coronary heart disease,combining with the degree of coronary artery stenosis reported by coronary angiography(assessing stenosis degree by calculating Gensini score),and to understand the correlation between platelet activation and coronary heart disease.Method:Collected patients who first visited the Department of Cardiovascular Medicine of Shaanxi People's Hospital between March 2018 and December 2018.After excluding other factors that significantly affected platelet activation,we included patients who met the criteria for enrollment.According to the results of inquiry history,detailed physical examination,electrocardiogram,laboratory assistant examination and coronary angiography,81 cases were selected as the experimental group,and 19 cases as the control group were selected as normal healthy people(<40y)or patients who were excluded from the diagnosis of coronary heart disease by coronary CTA and coronary angiography.After admission,all patients underwent detailed medical history inquiry and physical examination,and recorded clinical data such as age,sex,blood pressure and so on.The platelet activation function was tested before antiplatelet drugs were administered.All the patients in the experimental group were examined by emergency or selective coronary angiography according to the judgement of the doctor,and Gensini score was made according to the results of coronary angiography to judge the degree of coronary artery stenosis in the experimental group.Subgroups of patients with different types of coronary heart disease in the experimental group were compared.Result:1.Data comparison between experimental group and control group:In the comparison of general clinical data,there was no significant difference in age,sex,blood pressure,liver function,creatinine and other factors(P > 0.05).There were significant differences between LDL and LV ejection fraction(P < 0.05).The results of platelet activation function test showed that the results of platelet activation function test(CD62P,PAC-1)in the experimental group were higher than those in the control group,and there was a significant difference between the two groups(P < 0.05).2.Data comparison of coronary heart disease subgroup:The expression rate of(CD62P/CD61+)in the three groups was AMI(6.35±2.60)>UAP(5.20±1.97)>SAP(4.76±2.02)(P < 0.05).The expression rate of(PAC-1/CD61+)in the three groups was AMI(11.54±3.45)>UAP(9.66±3.10)>SAP(7.99±2.87)(P < 0.05).3.Spearman correlation analysis showed that platelet activation level was positively correlated with the degree of coronary artery stenosis(Gensini score).4.Linear correlation analysis showed that there was a positive correlation between platelet activation level and MPV in patients with coronary heart disease.5.Logistic regression analysis showed that family history of coronary heart disease and early platelet activation were risk factors for acute coronary syndrome.Conclusion:1.The level of platelet activation in patients with coronary heart disease is higher than that in normal people.2.There were differences in platelet activation levels among patients with different types of coronary heart disease,and the degree of coronary stenosis(Gensini score)increased with the increase of platelet activation level.
