ObjectiveTo observe the clinical effect of Jiawei Shiquan Decoction combined with enteral nutrition on syndrome integral, nutritional index, lung function and diaphragm activity of AECOPD patients with deficiency of both qi and blood in malnutrition,so as to provide more reference for the prevention and treatment of AECOPD patients with malnutrition.MethodsSixty patients with AECOPD combined with malnutrition syndrome were divided into experimental group(n=30)and control group(n=30)by random grouping method. Both groups received routine western medicine treatment and enteral nutrition intervention. The experimental group was treated with Jiawei Shiquan Decoction,1 dose a day, decocted to 400 ml in the morning and evening.Take warm clothes after two meals for 10 days.Syndrome score, nutritional index(BMI),skin fold thickness(TSF),upper arm midpoint muscle circumference(MAMC),serum albumin(ALB),hemoglobin(HGB),lung function(FEV1),forced vital capacity(FVC),one second rate(FEV1/FVC),diaphragm activity(DEQB,DEDB)were observed before and after treatment.Results(1)Before treatment, there was no difference between the two groups in syndrome scores(P>0.05);after treatment, the syndrome scores of the two groups decreased significantly compared with the control group, and the difference was statistically significant(P<0.05).(2)Before treatment, there was no difference in serum ALB between the two groups(P>0.05);after treatment, the ALB levels of the two groups were higher than before, and improved significantly compared with the control group, the difference was statistically significant(P<0.05).Before treatment, there was no difference in serum HGB between the experimental group and the control group(P>0.05);after treatment, there was no significant change in serum HGB levels between the two groups compared with the former(P>0.05),and there was no difference between the two groups(P>0.05).(3)Before treatment, there was no difference in BMI,TSF and AMC between the two groups(P>0.05).After treatment, the levels of BMI,TSF and MAMC in the two groups had no significant changes compared with those before treatment, and there was no difference between the two groups(P>0.05),but there was no difference between the two groups(P>0.05).(4)Before treatment, there was no difference in FEV1,FVC,FEV1/FVC% between the two groups(P>0.05).After treatment, the levels of FEV1,FVC,FEV1/FVC% in the two groups were higher than before, and the improvement was obvious compared with the control group, the difference was statistically significant(P<0.05).(5)Before treatment, there was no difference in DEQB and DEDB between the two groups(P>0.05);after treatment, the diaphragm activity during quiet breathing decreased significantly in both groups compared with the control group(P<0.05).After treatment, the diaphragm activity of the two groups during forced breathing increased significantly compared with the control group(P<0.05).ConclusionJiawei Shiquan Decoction combined with enteral therapy can improve the nutritional status and respiratory muscle function of AECOPD patients with deficiency of both qi and blood, alleviate clinical symptoms effectively and improve syndrome scores;the effect is better than that of enteral nutrition alone.
