Clinical Observation On The Treatment Of AECOPD Complicated With Malnutrition In Elderly Patients (Qi Deficiency Of Lung And Spleen) With Modified Huangqi Jianzhong Decoction Combined With Enteral Nutrition

Objective:Using modified Huangqi Jianzhong Decoction and enteral nutrition treat the elderly malnourished patients with AECOPD and observing the efficacy.To explore the effect of modified Huangqi Jianzhong Decoction and to provide clinical methods for the prevention and treatment of elderly AECOPD malnutrition with integrated traditional Chinese and western medicine.Method:Diving 70 patients who passed the screening into treatment group(n=35)and control group(n=35)by using the randomized controlled trial method.Western medicine treatment was the basic treatment,the control group was treated with enteral nutrition powder,and the treatment group was treated with modified Huangqi Jianzhong Decoction and enteral nutrition powder,and the intervention was two weeks.Comparing the the level of albumin and prealbumin,TCM syndromes scores,GSRS scores,and CAT scores of the two groups before treatment,1 week of treatment,and 2 weeks of treatment,and comparing the body mass index(BMI)of the two groups before and after treatment,then making a statistical analysis by using the reaserch data.Result:(1)Total effective rate:the treatment group was 91.2%,the control group was79.4%(P<0.05).The treatment group’s efficacy was better.(2)Nutritional indicators:BMI changes in the two groups before and after treatment were not statistically significant(P>0.05).The changes of albumin and prealbumin were statistically different between the two groups(P<0.05).(3)TCM syndromes:After treatment,the total score of two groups decreased,the decreased scores of the treatment group were more statistically,and there was a statistical difference between the two groups(P<0.05).After treatment,the treatment group showed significant improvement in cough,expectoration,shortness of breath,fatigue,lack of appetite,abdominal distension,and loose stools,and the difference was statistically significant(P<0.05).There was no statistically significant difference in susceptibility to colds,spontaneous sweating and yellow complexion(P>0.05).In the control group,after treatment,there was statistically significant difference in cough,expectoration,shortness of breath(P<0.05),there was no statistically significant difference in gastrointestinal symptomsis,susceptibility to colds,spontaneous sweating,and yellow complexion(P>0.05).(4)GSRS score:There was no significant difference in the total score of the control group(P>0.05).The total score of the treatment group was lower than before treatment,the improvement of gastrointestinal symptomsis was obvious,the difference was statistically significant(P<0.05).(5)CAT score:After treatment,the CAT scores of the two groups decreased,there was a statistical difference within the two group(P<0.05),and the treatment group had better effect(P<0.05).Conclusion:Modified Huangqi Jianzhong Decoction combined with enteral nutrition can improve nutritional status,gastrointestinal symptoms,respiratory symptoms and quality of life of elderly AECOPD malnourished patients.Modified Huangqi Jianzhong Decoction combined with enteral nutrition has more excellent clinical efficacy than simple enteral nutrition.
