| ObjectiveThe clinical and pathological data of 328 cases of epithelial ovarian cancer in Sichuan Provincial people’s Hospital were retrospectively analyzed.The survival was followed up to explore the factors affecting the prognosis of epithelial ovarian cancer,and to compare the clinical characteristics of epithelial ovarian cancer with different tissue subtypes.To provide a reference for the treatment of epithelial ovarian cancer with different tissue subtypes.Materials and MethodsFrom January 2008 to December 2015,328 patients with epithelial ovarian cancer underwent surgical treatment in Sichuan Provincial people’s Hospital.The clinical data,such as age of onset,preoperative CA125 level,surgical methods and so on,were collected and followed up until July 2017.Pathological sections of 328 patients with ovarian cancer were re-sectioned and stained with hematoxylin-eosin(hematoxylin-eosin staining,HE)and immunohistochemical(Immunohistochemistry,IHC),according to the 2014 World Health Organization(WHO)the fourth edition of the classification criteria for epithelial ovarian tumors classified all tumors,compared the clinicopathological features of each type of ovarian cancer,and discussed the factors affecting the prognosis of epithelial ovarian cancer.Apply SPSS 22.0 statistical software for statistical analysis.The Kruskal-Wallis test of rank sum test was used for age comparison between groups,and the results were expressed as median(quartile).Chi-square test was used to compare the FIGO stage,preoperative CA125 value and chemotherapy among the groups.The total survival time(Overall survival,OS)and progression-free survival time(Progression free survival,PFS)were analyzed by Kaplan-Meier method.Log-rank test was used to compare the effects of FIGO stage,preoperative CA125 value and chemotherapy on survival time.COX regression modelwas used to analyze the effect of multiple factors on survival time(P < 0.05).05 the difference is statistically significant.ResultsAs of July 2017,152(46.3%)of 328 patients with epithelial ovarian cancer were diagnosed in Sichuan Provincial people’s Hospital.The median survival time was 53 months.The 5-year progression-free survival rate of patients with epithelial ovarian cancer was about 30%.The total 5-year survival rate was about 45.FIGO stage,preoperative CA125 level(P < 0.001)and survival time(PFS:P=0.004;)of ovarian cancer with different tissue subtypes(P < 0.001)The progress free survival and total survival were shorter in patients with late FIGO staging(P < 0.001),and shorter in patients with higher CA125 level before operation(P < 0.001).ConclusionThe clinicopathological features and prognosis of ovarian cancer with different tissue subtypes were different.High-grade serous ovarian cancer was found most in the late stage of the disease.The preoperative CA125 was higher and sensitive to platinum-based chemotherapy,but the five-year survival rate was low;Clear cell ovarian cancer most was found in the early stage of the disease,but it is not sensitive to chemotherapeutic drugs,with short progression-free survival time,high recurrence rate,low five-year survival rate and poor prognosis.Endometrioid ovarian cancer is sensitive to chemotherapy,with long progression-free survival,high five-year survival rate and best prognosis.FIGO stage,preoperative CA125 level and response to chemotherapeutic drugs are all factors affecting the prognosis of ovarian cancer.FIGO staging is late.High preoperative CA125 levels and poor prognosis for chemotherapy-resistant patients always have poor prognosis. |
