Objective:To analyze the relationship between preoperative HBV-DNA load in patients with hepatic alveolar echinococcosis complicated with chronic hepatitis B and blood routine, coagulation function, liver function index and postoperative complications before and after surgery. To study the relationship between HBV-DNA load and the prognosis of patients with hepatic alveolar echinococcosis, and to provide a basis for the timing of surgery for patients with hepatic alveolar echinococcosis and chronic hepatitis B.Methods:The data of patients with hepatic alveolar echinococcosis complicated with chronic hepatitis B who were diagnosed and underwent surgical treatment in our center from January 2014 to July 2018 and met the inclusion criteria were collected as the experimental group. The data of 20 patients with hepatic alveolar echinococcosis treated by surgery during this period were randomly collected as the control group.Measuring and comparing the liver function index, the coagulation function index between the experimental group and control group in preoperative and postoperative 1,3,7days of and comparing the two groups of patients with postoperative complications occurred; Then, according to the HBV-DNA load level, the experimental group was divided into three groups,A,BandC,and the indicators and complications of different HBV-DNA load groups were compared,so as to obtain the relationship between HBV-DNA load and the prognosis of patients undergoing surgical treatment for hepatic alveolar echinococcosis. Results:During this period,a total of 625 patients with hepatic alveolar echinococcosis were collected, including 126 patients with chronic hepatitis B,accounting for 19.53%,and 28 patients meet the inclusion criteria.Comparing the experimental group with control group:serum total bilirubin at 7 days postoperatively, serum direct bilirubin at 3 days and 7 days after surgery, serum prothrombin time at 1 day after surgery, postoperative complications and hospital costs were significantly different between the two groups(P<0.05),multivariate analysis, whether infected with hepatitis B and lesion size were independent risk factors for postoperative complications of patients with hepatic alveolar echinococcosis.Comparison of the three groups A,B and C,there was no significant difference in postoperative liver function indexes and complications among the three groups(P>0.05).Conclusions:1.Hepatitis B virus infection has a certain impact on liver function indicators and hospital costs and complications of surgical treatment for patients with hepatic alveolar echinococcosis.2.Preoperative infection with hepatitis B and lesion size were independent risk factors for postoperative complications.3.According to the current data, the level of HBV-DNA load has no influence on the surgical treatment of patients with hepatic alveolar echinococcosis.
