Objective:In this paper,we aims to study the changes of T cell subsets in elderly patients with herpes zoster(HZ).To study the relationship between these changes and pain levels and the postherpetic neuralgia(PHN).Methods:1.From August 2017 to October 2018,forty-nine elderly patients with herpes zoster were selected as the herpes zoster group(HZ group),who were clinically diagnosed and admitted to the dermatology department of Qinghai Provincial People's Hospital.2.Twenty-two peers who undergo healthy physical examination at the same time were randomly selected as healthy controls(control group).3.T cell subsets(CD3~+T%,CD4~+T%,CD8~+T%and CD4~+T/CD8~+T)were measured by flow cytometer.4.The visual analogue scale(VAS)was used to determine the pain score of patients with herpes zoster.According to the score,the HZ group was divided into three subgroups:mild pain group,moderate pain group and severe pain group.5.The patients with herpes zoster were followed up for postherpetic neuralgia a month later.According to the follow-up results,the component was divided into two subgroups:the postherpetic neuralgia group(PHN group)and the non-postherpetic neuralgia group(non-PHN group).6.To analyze the relationship between T cell subsets and pain levels and postherpetic neuralgia in elderly patients with herpes zoster.Results:1.The CD3~+T%,CD4~+T%and CD4~+T/CD8~+T in HZ group were lower than those in the control group,and the CD8~+T%in HZ group was higher than that in the control group,the difference was statistically significant(P<0.05).2.Differences in T cell subsets between the three groups in the mild,moderate,and severe pain groups.The CD3~+T%,CD4~+T%and CD4~+T/CD8~+T were higher in the mild pain group than in the moderate pain group than in the severe pain group,and the difference was statistically significant(P<0.05).3.The degree of pain(VAS score)was negatively correlated with the CD3~+T%,CD4~+T%and CD4~+T/CD8~+T,and positively correlated with the CD8~+T%.4.The CD3~+T%,CD4~+T%and CD4~+T/CD8~+T in PHN group were lower than those in non-PHN group,and the difference was statistically significant(P<0.05).The difference of the CD8~+T%in both groups was not statistically significant(P>0.05).Conclusions:1.The CD3~+T%,CD4~+T%and CD4~+T/CD8~+T in elderly HZ patients is lower than those of healthy peers,and the CD8~+T%in elderly HZ patients is higher than that of healthy peers.2.The more the CD3~+T%,CD4~+T%and CD4~+T/CD8~+T decrease in elderly HZ patients,the more painful they are.3.The CD4~+T%is closer to pain of elderly HZ patients than the other T cell subsets.The main immunological change in patients is the reduction in the CD4~+T%,and the primary immunological change in patients is the decrease in the CD4~+T%.4.The more the CD3~+T%,CD4~+T%and CD4~+T/CD8~+T in elderly HZ patients,the more likely PHN occurs in the later period.
