| Objective:To investigate the clinical data of patients with deep burn of hand treated in our department in the past two years by using the integrated diagnosis and treatment mode of Chinese and Western medicine and the traditional diagnosis and treatment mode,so as to compare the different diagnosis and treatment modes in wound healing time,scar growth and total angle of finger joint The difference of motor,TAM)and Carroll's upper limb function evaluation test.To judge whether the treatment effect of integrated diagnosis and treatment mode of traditional Chinese and Western medicine is better than that of traditional diagnosis and treatment mode,so as to provide certain clinical reference and suggestions for patients with deep burn of hand when choosing diagnosis and treatment mode.Methods:This project consists of two groups,one is research 1 and the other is research 2.In Study 1,the patients were divided into treatment group 1 and control group 1.The burn depth of the hand test wound was deep ? degree shallow wound,and the wound treatment was non-surgical treatment;in Study 2,the patients were divided into treatment group 2 and control group 2.The burn depth of the hand test wound was deep ? degree deep wound and ? degree wound,and the wound treatment was surgical treatment.Patients in treatment group 1 and 2 were treated with integrated diagnosis and treatment mode of integrated traditional Chinese and Western medicine;patients in control group 1 and 2 were treated with traditional diagnosis and treatment mode.According to the inclusion and exclusion criteria,the patients with deep burn of hand who were hospitalized in the burn department of Wuxi integrated traditional Chinese and Western medicine hospital from December 2017 to December 2019 were selected as the study objects in the two groups.The patients in the control group 1 and the control group 2 were treated with routine wound healing training after operation.At the same time,for the patients in treatment group 1 and treatment group 2,according to the different stages of these patients' condition and course of disease,according to the concept of holistic view of traditional Chinese medicine,our department takes early stress treatment and organ function protection as the starting point,wound treatment as the key point,and integrates comprehensive rehabilitation treatment into the whole process of diagnosis and treatment,and adopts active and effective measures for wound closure,scar prevention and treatment,and functional recovery There are many kinds of treatment methods,such as wound covering treatment,comprehensive sports treatment,physical factor treatment,psychological treatment,pressure treatment and so on.The time of wound healing was recorded.Six months after the treatment,the scar growth,finger joint activity and overall hand function were evaluated according to Vancouver Scar Scale(VSS),total active finger joint activity score and Carroll upper extremity function evaluation test.Statistical analysis was used to evaluate the therapeutic effect of the two modes.Results:According to the inclusion and exclusion criteria,80 patients participated in the trial,including 20 in control group 1(group A),20 in treatment group 1(group B),20 in control group 2(Group C),and 20 in treatment group 2(Group D).In Study 1,there were 11 males and 9 females in group A,13 males and 7 females in group B,8 males and 12 females in group C,10 males and 10 females in Group D.there was no significant difference in age,gender composition,burn area of hand and total burn area between study 1 and Study 2(P>0.05).1.Comparison of wound healing timeThe wound healing time of patients in the control group 1(group A)was(20.05 ± 1.791)days,and the wound healing time of patients in the treatment group 1(group B)was(15.25 ±1.743)days,and the difference was statistically significant(t=8.588,P<0.01).The wound healing time of patients in the control group 2(group C)was(30.35 ± 3.265)days,and that of the treatment group 2(group D)patients was(22.25± 3.354)days.The difference was statistically significant(t=7.739,P<0.01).2.Comparison of scar growthThe Vancouver Scar Scale score of the control group 1(group A)was(6.45 ± 1.395),and that of the treatment group 1(group B)was(3.55 ± 1.099).The difference was statistically significant(t=7.304,P<0.01).The Vancouver Scar Scale score of the control group 2(Group C)was(8.05± 1.191),and that of the treatment group 2(Group D)was(6.15 ± 1.137).The difference was statistically significant(t=5.161,P<0.01).3.Comparison of total active mobility of finger jointIn the control group 1(group A),the score of total active activity of finger joint was(2.45±0.510),while in the treatment group 1(group B),the score of total active activity of finger joint was(3.30 ± 0.657),the difference was statistically significant(t=-4.569,P<0.01).In the control group 2(Group C),the score of total active activity of finger joint was(1.95±0.605),and in the treatment group 2(Group D),the score of total active activity of finger joint was(3.15 ± 0.745),the difference was statistically significant(t=-5.592,P<0.01).4.Comparison of overall functions of handThere was a significant difference between the control group 1(group A)and the treatment group 1(group B)in the evaluation of the overall hand function between the two groups(Z=-2.459,P=0.014).There was a significant difference between the control group 2(Group C)and the treatment group 2(Group D)in the evaluation of the overall hand function between the two groups(Z=-2.043,P?0.041).Conclusion:The integrated diagnosis and treatment mode of traditional Chinese and Western medicine is better than the traditional mode in the treatment of hand deep burn wounds.Compared with the traditional treatment mode,the integrated diagnosis and treatment mode of traditional Chinese and Western medicine can reduce the wound healing time,reduce the scar hyperplasia,increase the joint activity and improve the overall function of the hand,which is a treatment mode worthy of clinical promotion. |
