BackgroundWith the development of population aging and the increasing maturity of coronary heart disease interventional surgery technology,the incidence,disability and mortality of chronic heart failure are increasing.Therefore,more and more attention is paid to its prevention and treatment.In clinical practice,the search for markers that can evaluate changes in cardiac function in the course of chronic heart failure is helpful for accurate judgment of CHF.In recent years,the soluble matrix lysin-2(soluble ST2,sST2),Homocysteine(Homocysteine,Hcy)and soluble tyrosine kinase receptor AXL(soluble AXL,sAXL)factor is more and more attention from researchers,their three predictive value in diagnosis and treatment of heart failure are obtained the high evaluationObjectiveThis study intends to test and analyze the levels of new biomarkers sST2,Hcy and sAXL found in patients with chronic heart failure in recent years,in order to study the diagnosis and risk prediction of sST2,Hcy and sAXL in patients with heart failure.This aspect provides theoretical basis for its clinical applicationMethods120 patients with confirmed chronic heart failure who were admitted to our hospital's cardiology department or inpatient from January 2018 to December 2018 were selected as the case group,and the patients were classified according to the NYHA classification standard of the New York Heart Association of the United States.Select 120 healthy people who went to the hospital for physical examination during the same period as the healthy control group;ELISA double antibody sandwich method was used to measure the concentration of NT-pro BNP,sST2 and sAXL,and cyclic enzyme method was used to measure Hcy level;Philip-si E33 cardiac color Doppler ultrasound LVEF and LVEDd were measured in patients;index correlation analysis was performed using Pearson method;gene analysis of sST2,Hcy,and sAXL factors was performed using GO analysis and String database tools Search for functions and related proteins to further analyze their functionsResultsComparative analysis of sST2,Hcy,sAXL,NT-pro BNP,and cardiac function indicators LVEF and LVEDd in patients in the CHF group and the control group showed that except for the LVEF value in the CHF group,which was significantly lower than the control group,the other indicators in the CHF group were significantly higher than the control group,the differences were statistically significant(P<0.05).The test results of sST2,Hcy,sAXL,and NT-proBNP between the subgroups in the CHF group showed that the indicators in the NYHA ? group were significantly higher than those in the NYHA? group,and the differences were statistically significant(P<0.05);The indexes were significantly higher than those in NYHA ? group and NYHA ? group,and the difference was statistically significant(P<0.05).Correlation analysis showed that sST2 was correlated with NT-proBNP,LVEF,LVEDd,Hcy,and sAXL,among which it was negatively correlated with LVEF and positively correlated with other indicators;Hcy was correlated with NT-proBNP,LVEF,LVEDd,sST2,and sAXL.Correlation,which is negatively correlated with LVEF and positively correlated with other indicators;sAXL is correlated with NT-proBNP,LVEF,sST2,and Hcy,among which it is negatively correlated with LVEF,and there is no significant difference in correlation with LVEDd,with other The indicators are positively correlated.GO analysis analyzed the genes IL1RL1 and AXL corresponding to sST2 and sAXL,and the results showed that both played important roles in biological processes,molecular functions and cellular components.String database analysis revealed that the proteins interacting with sST2(IL1RL1)include Interleukin 33(IL-33)?Interleukin 13(IL13)?Interleukin 4(IL4)?GATA binding protein 3(GATA3)?IL-1 receptor-associated kinase 1(IRAK1)?IL-1 receptor-associated kinase 4(IRAK4)?Tumor necrosis factor receptor-associated factor 6(TRAF6)?Myeloid Differentiation Factor 88(MYD88)?Transmembrane Emp24 Domain Containing Protein 1(TMED1)?Interleukin-1 receptor accessory protein(IL1RAP).Among them,IL33,IL13,and IL4 are all related to the inflammatory response,which are mutually corroborated with the above GO analysis results.Interaction proteins with AXL include Src homology 2domain containing transforming protein 1(SHC1)?Kinase insert domain-containing receptor(KDR)?Phosphoinositol-3 kinase Catalytic subunit 8(PIK3C8)?Phosphatidylinositol 3-kinase regulates subunit 2(PIK3R2)?Phosphatidylinositol 3-kinase regulates subunit 1(PIK3R1)?Phosphoinositol-3 kinase Catalytic subunit ?(PIK3CA)?Milk fat globule-EGF factor 8(MFGE8)?Fibronectin 1(FN1)?Hygroscopic growth factors(HGF)?Vascular Endothelial Growth Factor A(VEGFA)?Interleukin 6(IL6)ConclusionsThe serum levels of sST2,Hcy,and sAXL in patients with CHF are significantly higher than those in healthy subjects,and increase with the deterioration of cardiac function.At the same time,there is a certain correlation with NT-proBNP and cardiac function ultrasound indicators.Therefore,sST2,Hcy and sAXL have certain clinical value in the diagnosis and severity assessment of patients with CHF.
