Background:Syphilis is a chronic and systematic sexually transmitted disease caused by Treponema pallidum(T.pallidum),which is seriously harmful to human health.The course of syphilis is complex,the clinical symptoms are diversified,and multiple organs are involved,which is very confusing.Clinically,some syphilis patients will have the phenomenon of serofast after the regular treatment.The patients with serofast can not monitor the curative effect and evaluate the prognosis according to rapid plasma reagin(RPR),so the clinical treatment is very difficult.IL-1?is a potent intracellular inflammatory factor.IL-1?can promote the innate immune system to eliminate pathogens by participating in immune regulation and inflammatory response.Studies have shown that after Treponema pallidum infection,the adhesion protein Tp0663 can induce THP-1 cells to secrete proinflammatory cytokines such as IL-1?and TNF-?.IL-1?can promote the differentiation of CD4~+T cells into Th17subsets and induce CD8~+T cells to secrete cytokines to assist cellular immunity.IL-8 is an important neutrophil chemokine,which can guide neutrophils to gather to the inflammatory site,and release harmful substances such as lysosomal enzymes,resulting in local inflammation and systemic symptoms of Tp infection.In our previous research work,we found that the outer membrane protein Tp92 of Treponema pallidum can regulate the secretion of IL-8 by THP-1 cells through NF-?B pathway.Therefore,we speculate that both IL-1?and IL-8 may be involved in the inflammatory response after Tp infection.At present,the cellular immune mechanism of patients with Tp infection is not fully understood.The study on the levels of serum IL-1?and IL-8 in patients with Tp infection along with the changes of IL-1?and IL-8 levels in patients with syphilis serofast before and after treatment may provide some experimental basis for the development as well as prognosis of syphilis,and provide new thinking for exploring the pathogenesis and efficacy monitoring of syphilis serofast.Objective:The serum expression levels of IL-1?and IL-8 in patients with untreated primary syphilis,secondary syphilis and latent syphilis were detected to analyze the correlation between serum IL-1?and IL-8expression levels and clinical stages in patients with Tp.To study the changes of serum IL-1?and IL-8 expression in patients with syphilis serofast before and after standard treatment,and to explore the correlation between IL-1?,IL-8 and syphilis serofixation.Methods:1.From May 2018 to January 2020,24 patients with primary syphilis,55 patients with secondary syphilis and 41 patients with latent syphilis were collected in the first people's Hospital of Changde City,and40 healthy subjects were selected as a normal control group.The expression levels of IL-1?and IL-8 in serum samples of each group were detected by double antibody sandwich enzyme-linked immunosorbent assay,and the expression levels of IL-1?and IL-8 in each group were compared.2.60 syphilis serofast patients who received standard treatment in this hospital in the same period were selected,and the expression levels of serum IL-1?and IL-8 before and after treatment were measured and compared,and the correlation analysis was carried out.Results:1.The expression levels of serum IL-1?and IL-8 in primary syphilis group,secondary syphilis group and latent syphilis group were significantly higher than those in normal control group(P<0.01),and the expression levels of IL-1?and IL-8 in secondary syphilis group were significantly higher than those in primary syphilis group and latent syphilis group(P<0.05).2.There was a positive correlation between serum IL-1?and IL-8expression levels in secondary syphilis group(r=0.390,P<0.01).There was no correlation between serum IL-1?and IL-8 expression levels in primary syphilis group and latent syphilis group(P>0.05).3.There was a positive correlation between the expression level of serum IL-8 and the titer of TRUST in secondary syphilis group(r=0.617,P<0.01),but there was no correlation between serum IL-1?expression level and serum TRUST titer in patients with secondary syphilis(P>0.05).There was no correlation between serum IL-1?and IL-8 expression levels and serum TRUST titer in primary syphilis group and latent syphilis group(P>0.05).4.The serum IL-8 expression level of patients with syphilis serofast was significantly lower than that before treatment(P<0.001),but there was no significant difference in serum IL-1?expression level before and after treatment(P>0.05).The expression levels of serum IL-1?and IL-8in patients with syphilis serofast were significantly higher than those in normal controls before treatment(P<0.001).There was no correlation between serum IL-1?and IL-8 expression levels in patients with syphilis serofast before and after treatment(P>0.05).Conclusions:1.High levels of serum IL-1?and IL-8 were expressed in patients with primary syphilis,secondary syphilis and latent syphilis,indicating that the changes in serum IL-1?and IL-8 expression levels in patients with syphilis were related to the inflammatory response of against Tp infection.2.The expression levels of IL-8 in patients with syphilis serofast decreased significantly after treatment,which may be helpful to monitor the curative effect and judge the prognosis of patients with syphilis serofast.
