| Objective:To explore the therapeutic effect of café-au-lait spots treated by Q-switched alexandrite laser(755 nm),recurrence and the relationship between the influencing factors.Methods:496 patients who underwent café-au-lait spot treatment from January 2010 to January 2018 were selected as the research objects, and the Q-switched alexandrite laser(755 nm)was used for instrumentation.Through follow-up and consulting the outpatient medical records, the basic information of these patients,as well as the size, number, distribution, treatment effect, recurrence and complications of café-au-lait spots and other aspects of information were collected.According to the improvement of the lesion color, the therapeutic effect could be divided into four levels:ineffective, improved, effective and cured. The collected data was processed by SPSS22.0.Results:1、By analyzing the factors that affect the therapeutic effect and comparing café-au-lait spots with regular and irregular boundary,it was found that the effective rates of café-au-lait spots with regular and irregular boundary were 60.17%and 78.85%respectively,which was statistically significant(P<0.05);by comparing café-au-lait spots with even and uneven pigment, the effective rates of café-au-lait spots with even and uneven pigment were 64.05%and 79.47%respectively,which was statistically significant(P<0.05);the effective rates of small, medium and large lesions were 80.68%、66.67%and 61.29%,respectively;there was statistically significant difference between the small and the medium lesions(P<0.05);there was statistically significant difference between the small and the large lesions(P<0.05);there was no statistical correlation between the gender, age, the location of skin lesions, the color of skin lesions, the energy density, the number of treatment, the interval of treatment and the therapeutic effect(P>0.05).According to the correlation analysis of factors with statistical significance, the order of influence on the efficacy is:the area of skin lesions>the shape of skin lesions>the degree of pigment uniformity.2、By analyzing the factors that affect the therapeutic effect and comparing café-au-lait spots with regular and irregular boundary, the recurrence rate of café-au-lait spots with regular and irregular boundary was 48.73%and 39.62%,respectively(P<0.05);by comparing café-au-lait spots with even and uneven pigment, the recurrence rate of café-au-lait spots with even and uneven pigment was 48.04%and 37.37%respectively,which was statistically significant(P<0.05).The uniformity of pigment was closely related to the therapeutic effect. The recurrence rates of small, medium and large lesions were 30.11%,45.45%and 58.06%respectively,which had statistical significance(P<0.05).The recurrence rates of different therapeutic effects were 83.22%(ineffecti ve),46.79%(improved),19.51%(effective)and 14.86%(cured)respectively;the recurrence rate of ineffective patients was significantly higher than that of patients with other therapeutic effects, which was statistically significant(P<0.05);there was no statistical correlation between the color of skin lesions, location, energy density,.the number of treatment, treatment interval and recurrence(P>0.05).According to the correlation analysis of factors with statistical significance, the order of influence on recurrence is:curative effect>skin area>pigment uniformity>skin morphology. In this study,2 patients had long-term complications.Among them,l case was hypopigmentation,1 case suffered from chromatosis with secondary dermatitis.Conclusion:1、There is statistical correlation between the area, boundary and even pigment of café-au-lait spot and the therapeutic effect and recurrence. The café-au-lait spot with large area, regular boundary and uneven pigment have worse therapeutic effect and higher recurrence rate.2、There is a negative correlation between the therapeutic effect and the recurrence rate.3、It was preliminarily considered that the influence degree of the curative effect was as follows:lesion area>lesion shape>pigment uniformity. The influence degree of recurrence was:curative effect>lesion area>pigment uniformity>lesion shape. |
