| Objectives(1)To investigate the status of rehabilitation confidence of stroke patients through questionnaire survey.(2)Explore the influencing factors of rehabilitation confidence of stroke patients,and provide a reference for the construction of nursing intervention programs.(3)Establish a nursing intervention program for rehabilitation confidence of stroke patients.Methods A convenient sampling method was used to select 235 stroke patients who were hospitalized in the Department of Neurology in a tertiary Class A hospital in Beijing from December 2018 to July 2019.The general information questionnaire,Confidence after Stroke Measure,Stroke Self-Efficacy Questionnaire,Self-Esteem Scale,Self-Rating Depression Scale and Medical Outcomes Study Social Support Scale were used to investigate stroke patients.The t-test and one-way analysis of variance were used to compare the differences in rehabilitation confidence among stroke patients with different demographic characteristics.Pearson correlation analysis was used to analyze the correlation between rehabilitation confidence and self-efficacy,self-esteem,depression,and social support levels in patients with stroke.Multivariate linear regression analysis was used to construct a model of influencing factors of rehabilitation confidence in stroke patients,and to determine the main influencing factors of it.Delphi method was used for expert consultation to verify the reliability and scientificity of the rehabilitation confidence nursing intervention program for stroke patients.Results(1)The average rehabilitation confidence score of 235 stroke patients was(47.63±11.00),and the standardized score rate was 58.80%,which was at a low level.The three dimensions are ranked in descending order of standardized scores as positive attitude,social confidence,and self-confidence.(2)Analysis of influencing factors of rehabilitation confidence after stroke show that patients with different age,marital status,education,occupation,family monthly income,living status,type of escort,personality type and number of incidences have statistically significant differences in rehabilitation confidence(P<0.05).Pearson correlation analysis showed that the dimensions and total scores of stroke rehabilitation confidence were positively correlated with self-efficacy,self-esteem and social support,and negatively correlated with depression.Multivariate linear regression analysis showed that depression,social support,intermediate personality and self-esteem are the main influencing factors of rehabilitation confidence of stroke patients,which explained 51.7%of the total variation of rehabilitation confidence of stroke.(3)After two rounds of expert consultations in this study,the opinions of the experts reached an agreement.The positive coefficients of the experts in the two rounds were 88.89%and 100%,both of which were greater than 70%,and the enthusiasm of the experts was very high.The expert authority coefficient is 0,87±0.09 as a whole,which is greater than 0.70,and has good authority.In the two rounds of expert consultation,the coefficient of variation of each item was less than 0.25,the degree of variation was low;the coordination coefficients were 0.36 and 0.43,and the P values were less than 0.05,which was statistically significant and the coordination of expert opinions was good.Conclusions(1)The rehabilitation confidence of stroke patients is at a low level and needs to be further improved.(2)Age,marital status,education,occupation,family monthly income,living status,type of escort,personality type and number of incidences can affect the rehabilitation confidence of stroke patients.Among them,depression,social support,intermediate personality and self-esteem level are the main influencing factors of rehabilitation confidence in stroke patients.It is suggested that different intervention methods should be given to patients with different demographic characteristics and psychosocial characteristics in the nursing work,and targeted nursing intervention program should be formulated.Among them,we should focus on the main influencing factors and make targeted adjustments and improvements.Emphasize the importance of family members of patients,encourage and guide family members to participate in the physical rehabilitation and psychological construction of patients,give play to their role of support and supervision,in order to enhance patients’confidence in rehabilitation,promote comprehensive rehabilitation and improve prognosis.(3)The consultation results of the rehabilitation confidence nursing intervention program for stroke patients constructed in this study are reliable and the program is scientific. |
