Clinical Retrospective Analysis On 280 Patients With Behcet's Disease

Objective: Through retrospective analysis on clinical data of 280 Becet's disease patients,to summarize clinical features of Becet's disease patients in our hospital,further understand the clinical features and pathogenesis of Becet's disease and improve the clinical diagnosis and treatment level.Method: Data of Becet's disease patients who were hospitalized and visited department of stomatology in the First Affiliated Hospital of Dalian Medical University from November 2009 to November 2019 were collected,including name,gender,nationality,region,age(age of first attack,age at diagnosis,age of hospitalization,age of appearance of clinical features),previous history,family history,initial symptoms,first department,first clinical symptoms,pre-admission diagnosis,pathogenesis and inducement,course of disease(time from initial symptoms to this visit,time from initial symptoms to appearance of clinical features),clinical features,laboratory examination,histopathology examination,etc.,SPSS20.0 software was used for statistical analysis.Result: Of 280 Becet's disease patients,95 were male(33.93%)and 185 were female(66.07%).The ratio of male to female was 1:1.95.The age of attack ranged from 4 years to 84 years,with a median of 34.00 years,and 20-40 years(52.14%)were more common.The median age at diagnosis was 37.00 years.Incidence of oral ulcer was97.86%,which was the most common clinical manifestation,and that of genital ulcer was 80.36%.In terms of genital ulcer,the average age of attack of female was34.88±13.04 years and that of male was 40.04±16.00 years.The age of female genital ulcer was earlier than that of male(P<0.05).Other clinical manifestations were 57.14%of skin lesions(erythema nodosum,folliculitis,rash,pimples,maculopapule,etc.),27.86% of joint involvement,20.36% of eye involvement,13.57% of digestive system involvement,8.93% of heart involvement,6.79% of renal involvement,6.79% of thyroid involvement,6.07% of nervous system involvement,5.36% of vascular involvement,4.29% of pulmonary involvement,3.75% of blood system involvement.Among first clinical symptoms,oral ulcer or oral ulcer merge other symptoms was the most common.Among first departments,dermatology department or rheumatology and immunology department was the most common.In laboratory examination,54.39% of increased C-reactive protein(CRP),55.74% of increased Erythrocyte Sedimentation Rate(ESR),14.84% of increased White Blood Cell(WBC),15.23% of increased Neutrophil ratio(NEUT),21.36% of Antinuclear antibody(ANA)positive,16.04% of increased Immunoglobulin G(Ig G),10.38% of increased Immunoglobulin A(Ig A),3.32% of increased Immunoglobulin M(Ig M),5.19% of increased C3,7.08% of reduced C3,6.10% of increased C4 and 8.29% of increased antistreptolysin O(ASO),and there was no significant difference in the positive rate of various indicators between male and female.The pathergy test was performed in 78 patients,with 44 positive patients(56.41%),15 males(19.23%),29 females(37.18%)and 6 suspected positive patients.Histopathology examination was performed in 46 patients,37 patients of skin tissue,8 patients of digestive tract mucosa,1 patient of vulvar mucosal tissue,pathological manifestation of skin tissue and vulvar mucosal tissue was vasculitis,mostly leukocyte fragmenting vasculitis,and pathological manifestation of digestive tract mucosal tissue was chronic inflammation or chronic inflammation with acute inflammation.Conclusion:1.Becet's disease was more common in young adults,and incidence was higher in female than in male.There was no significant difference in the age of attack between male and female,but the age of onset of female genital ulcers was earlier than that of men.2.Among initial symptoms of Becet's disease,oral ulcer or oral ulcer merge other symptoms was the most common.Among first departments,dermatology department or rheumatology and immunology department was the most common.3.Among Becet's disease patients,incidence of erythema nodosum was higher in female than in male,and incidence of rash,folliculitis,and vascular lesions was higher in male than in female.4.Compared with BD patients with genital ulcers,BD patients without genital ulcers are more prone to eye diseases.
