Objective: To study the ultrasonic testing of hitches in pregnancy uterine arterial blood flow mechanics index combined maternal serum expression(PLGF)concentration placenta growth factor predicts recurrent miscarriage(recurrent what abortions,RSA)value,in order to provide reference for clinical precision intervention plan formulation.Methods: Selection in August 2018 to August 2019 clinic in our hospital during the period of 102 cases of pregnant women,62 cases of recurrent abortion patients as observation group,in addition to choose 40 cases of normal pregnancy women as control group,aged 22 to 45 years old,11 ~ 17 weeks ago for all pregnant women in pregnancy uterine artery ultrasound examination and detection of pregnant women serum PLGF concentration in the same period,RSA patients and normal pregnant women were compared the value of PI and RI value of uterine artery,S/D value and the concentration of serum PLGF differences have statistical significance.At the same time,the pregnancy outcomes of the two groups of pregnant women were followed up to observe whether there were differences in pregnancy outcomes between the two groups.By drawing four tables to calculate the uterine artery blood flow mechanics parameters,the concentration of the maternal serum PLGF parameters of the joint and the uterine artery blood flow in the maternal serum PLGF concentration sensitivity,specificity and draw the receiver-operatingcharacteristic curve(receiver operator characteristic curve,the ROC curve)to analyze and compare each hemodynamic parameters and maternal serum PLGF concentration independent diagnosis efficiency,and uterine artery blood flow mechanics parameters combined with the concentration of maternal serum PLGF integrated diagnosis efficiency.Results:A total of 102 patients aged from 22 to 45 years old,with an average age of 30.78±4.67 years old,including 62 patients with RSA and 40 normal pregnant women.The mechanical parameters RI,PI and S/D of uterine arterial blood flow in RSA patients were significantly higher than those in normal pregnant women,with significant statistical differences(P<0.05).However,the serum PLGF concentration in RSA patients was lower than that in normal pregnant women,showing a significant statistical difference(P<0.05).With the history of 2 or more abortions as the diagnostic criteria for RSA,the uterine artery RI value=0.795,PI value =1.53 and S/D value =2.55 were selected as the diagnostic threshold,and the AUC were 0.928,0.886 and 0.889,respectively.The diagnostic sensitivity and specificity were 95.1%,87.5%,63.9%,70.0%,91.8% and 42.5%,respectively.The serum PLGF concentration in pregnant women was selected as the diagnostic threshold of 51.70pg/ml,and the diagnostic sensitivity and specificity were 91.9%and 90.0%.The cut-off value of RI,PI and S/D combined with the serum PLGF concentration of pregnant women was 0.694,and the sensitivityand specificity of diagnosis were 96.9% and 97.5% respectively.Conclusion: The hemodynamic parameters RI,PI and S/D of uterine artery in patients with RSA were significantly higher than those in women without RSA history.Color doppler examination of uterine arterial blood flow during early pregnancy can help to detect changes in pregnant women's blood flow and predict the occurrence of RSA.The detection of serum PLGF concentrations in the two groups of pregnant women showed that the expression of PLGF in the serum of RSA patients was significantly lower than that in the serum of normal pregnant women.The detection of serum PLGF concentrations in the early pregnancy(11-17w)was helpful for the clinical prediction and diagnosis of RSA.In the early stages of pregnancy,the hemodynamic parameters of uterine artery can be accurately diagnosed with RSA by ultrasonography,the blood flow of uterine artery combined with the serum PLGF concentration in pregnant women can improve the accuracy and specificity of the diagnosis of RSA.Abnormal changes in uterine arterial blood flow mechanics parameters were found by color doppler ultrasound in the early second trimester of pregnancy.The combination of uterine artery blood flow parameters and maternal serum PLGF concentration can provide a new direction for clinical diagnosis and a better clinical treatment.
