Objective:Retrospectively analyze the changes of platelet-related and leukocyte-related parameters before and after bacterial infection.To analyze the changes of platelet-related and leukocyte-related parameters of Gram-positive and Gram-negative bacteria.To explore the role of platelet-related parameters in early diagnosis and prognosis of bacterial infection.Method:Collecting and organizing the data of inpatients in Zhujiang Hospital of Southern Medical University from January 2016 to April 2018,and screening out 201 cases of infections confirmed positive by bacterial culture(blood,sputum,urine,ascites,cerebrospinal fluid,drainage fluid,pus,secretions),78 cases(38.8%)of Gram-positive bacteria infections and 123 cases(61.2%)of Gram-negative bacteria were infected.The changes of parameters related to platelet and white blood cell before and after diagnosis of bacterial infection were observed and analyzed.Using spss22.0 statistical software analysis.Measurement data is expressed as meanąstandard deviation and count data is expressed as%,normality test is used SW test or KS test,difference comparison is used t test or wilcox rank sum test,Spearman correlation was used for correlation analysis,and the diagnostic efficacy was based on the receiver operating characteristic curve.Result:1.Gram-positive bacteria infection leads to increased PLT,PCT and PLT/LYM in patients,the degree of PLT increase is greater in men than women.When PLT is increased by 70.75×109/L-113.42×109/L,PCT is increased by 0.05%?0.32%,PLT/LYM is increased by 63.74?125.13,it is helpful to diagnose Gram-positive bacteria infection.The greater the degree of increase in WBC and GRA,the higher the risk of multiple organ failure,mortality,and the worse the prognosis.2.Gram-negative bacteria infection leads to a decrease in PLT and PCT and an increase in MPV and PLT/LYM.A decrease in PLT of 51.72 × 109/L?75.23 × 109/L,a decrease of PCT of 0.02%?0.17%,and an increase of PLT/LYM of 46.61?118.09,MPV increased by 0.04fL?0.36fL,which is helpful for the diagnosis of Gram-negative bacterial infections.Pseudomonas aeruginosa and Acinetobacter baumannii infections have significant differences in the decrease in PLT and PCT.The greater the decrease in PLT,the higher the risk of multiple organ failure in patients with Gram-negative bacteria infection.3.The degree of PLT and PCT increase in patients with Gram-positive bacteria infection is greater than that in patients with Gram-negative bacteria infection.Conclusion:Bacterial infection can cause significant changes in platelet count,platelet hematocrit,and the ratio of platelet count to absolute value of lymphocytes.The degree of change in platelet count,white blood cell count,and absolute value of lymphocytes will affect the prognosis of patients with bacterial infection.Different degrees of PCT and PLT changes can help distinguish between Pseudomonas aeruginosa and Acinetobacter baumannii infections.Before the positive result report of bacterial infection etiology test,combined with the white blood cell count and other indicators,the changes of platelet related parameters are helpful for early diagnosis and differential diagnosis,and guide the role of treatment.
