Objective:To investigate the correlation between antiphospholipid antibodies(aPLs)and ischemic stroke(IS)in patients with systemic autoimmune diseases(SADs).Methods:Forty-two SADs patients with IS were included as the case group(IS group),and 62 SADs patients without IS were included as the control group(non-IS group).Their peripheral blood aPLs expression levels were measured by enzyme-linked immunosorbent assay.Qualitatively compare the difference in positive rates of aPLs between the IS group and the non-IS group.Quantitatively compare the differences in aPLs expression levels between the two groups.And analyze the risk factors of IS in patients with SADs.Result:The positive rate of aPLs in IS group was significantly higher than non-IS group(61.9%vs.40.3%,?2=4.667,P=0.031).Comparing the expression levels of aPLs subtypes in the two groups,the expression levels of aCL-IgM(22.82±27.27 vs.13.34±8.43,t=-2.183,P=0.034)and a?2GPI-IgM(18.70±23.95 vs.7.61±5.80,t=-2.766,P=0.009)in the IS group were higher than the non-IS group.Compared to low aPLs patients,the prevalence of IS in high aPLs group increased remarkably(30.2%vs.64.7%,?2=6.472,P=0.011).The positive rate of aPLs in multiple IS group was significantly higher than non-IS group(81.8%vs.40.3%,?2=4.161,P=0.041).Moreover,the logistic regression analysis showed that aPLs were independent risk factors for IS in SADs patients.Conclusion:1.When aPLs are positive in SADs,the risk of IS will increase,especially in the presence of high serum concentrations of aPLs.2.The expression levels of aCL-IgM and a?2GPI-IgM are closely related to the occurrence of IS.3.Patients with aPLs-positive SADs complicated with IS are more common in multiple cerebral infarction lesions.APLs can be used as a predictive index to evaluate the degree of IS lesions in SADs.4.aPLs is closely related to the occurrence of IS in SADs,and is an independent risk factor for the occurrence of IS in SADs.
