Objective:To investigate the prognostic significance of platelets-to-lymphocyte ratio(PLR)in peripheral blood in patients with multiple myeloma(MM)Methods:Analyze retrospectively the clinical data of 116 newly diagnosed MM patients in our hospital from January 2009 to June 2019,select 100 cases of healthy subjects as control group at the same time,through the acquisition of the peripheral blood platelet and the absolute value of lymphocyte,calculate the PLR values,comparing the difference between the PLR value between the two groups,using the receiver-operating characteristic curve(receiver operating characteristic,ROC)to determine the value of PLR in a group of patients with MM best cutoff point namely cutoff value,according to a group of patients with MM cutoff value is divided into high PLR group(PLR 123 or higher)and low PLR(PLR<123),Then analysis of two groups of patients in gender,age,International Staging System(International Staging System,ISS),hemoglobin(Hb),serum calcium(Ca),serum beta 2 microglobulin(?2-MG)(propagated),serum albumin,serum creatinine(Cr),immune classification,lactate dehydrogenase(LDH)and bone related events(skeletal related events,SRE)differences in clinical features.Univariate and multivariate analyses were performed on the factors that might affect overall survival(OS)and progression free survival(PFS)of MM patientsResults:Group of patients with MM PLR values higher than the control group,the difference was statistically significant(t=2.034,p=0.043,<0.05),Compared with the low PLR group(PLR<123),the high PLR group(PLR? 123)had statistically significant differences in the ISS stage(?2=7.044,p=0.030)and ?2-MG(?2=4.878,p=0.027)clinical characteristics,single factor analysis showed that in patients with sex,age,ISS stage,Hb,Ca,?2-MG,Cr and PLR values on the prognosis of MM patients' PFS.However,ISS staging,Ca,?2-MG,Cr and PLR values were related to the prognosis of MM patients' OS.Further multivariate analysis showed that PLR values were independent risk factors for poor prognosis of MM patients' OS(HR:1.670,95%CI:1.028-2.713,p=0.038),while PLR values were not independent risk factors for MM patients' PFS(HR:1.353,95%CI:0.879-2.082,p=0.169).Conclusion:The PLR value of newly diagnosed MM patients can be used as an independent risk factor affecting the patient's OS.The current results suggest that the difference between the high and low PLR groups in OS and PFS is statistically significant,and PLR may be an effective indicator to predict the OS of MM patients.However,it is necessary for multicenter research institutions to establish a unified criteria for selecting the best cut-off point of PLR and Supporting the final result...
