ObjectiveBy observing the effects of nerve mobilization with different stretching parameters on motor nerve conduction velocity(MNCV),sciatic nerve function index(SFI),myeloid axon count and NGF protein expression in the corresponding sciatic nerve and spinal ganglia after sciatic nerve injury to determine the optimal stretching parameters for nerve mobilization,and provide more experimental support for clinical treatment.Methods1.Select 18 male SD rats and perform conventional left femoral skin preparation after anesthesia.Cut the skin,fascia,and muscle below the middle of the femur to find the sciatic nerve.Use two surgical lines to tie the sciatic nerve respectively.The lines are lcm apart.After artificially setting the limiting conditions,quickly walked along the sciatic nerve to do passive knee extension.When this segment of the sciatic nerve is extended to the required length,measure the knee angle at this time.Every 6 SD rats were used as a group,and the average was obtained after measuring 6 rats.These three angles are the angles required for subsequent treatment.2.90 SD rats were randomly divided into a normal control group,a model group,a 6%nerve mobilization group(6%of nerve stretching parameters),a 9%nerve mobilization group(9%of nerve stretching parameters),and a 12%nerve mobilization group(12%of nerve stretching parameters),18 rats in each group were divided into 3 subgroups according to 1 week,2 weeks,and 4 weeks after operation,and 6 rats in each subgroup.After grouping,the model is prepared.The motor nerve conduction velocity,sciatic nerve function index,myeloid axon counts of corresponding spinal ganglia and sciatic nerve,and expression of NGF proteins were measured at 1 week,2 weeks,and 4 weeks after surgery,respectively.Results1.Knee joint angle:The angle of the knee joint in the 6%group of nerve loosening was 100°;the angle of the knee joint in the 9%group of nerve loosening was 110°;the angle of the knee joint in the 12%group of nerve loosening was 120°.2.MNCV:After 1,2,and 4 weeks of treatment,the MNCV in the 6%of neuromobilization group,the 9%of neuromobilization group,and the 12%of neuromobilization group were higher than the model group and lower than the normal group(P<0.05).Compared with the three treatment groups of neuromobilization,the MNCV of 9%of neuromobilization group was higher than that of 6%of neuromobilization group and 12%of neuromobilization group.The difference was statistically significant(P<0.05).In the 12%of neuromobilization group,the difference was not statistically significant at the 1-week time point(P>0.05),and the difference was statistically significant at the 2-week and 4-week time points(P<0.05).Except for the normal group,the MNCV of each group peaked at 4 weeks3.SFI:After 1 week of treatment,the SFI in the 6%of neuromobilization group,the 9%of neuromobilization group,and the 12%of neuromobilization group were lower than the normal group,the difference was statistically significant(P<0.05);The SFI of 9%of neuromobilization group was higher than that of model group,and the difference was statistically significant(P<0.05).The SFI of 12%of neuromobilization group was higher than that of model group.After 2 weeks and 4 weeks of treatment,the SFI in the 6%group,9%group,and 12%group were all higher than the model group and lower than the normal group,the difference was statistically significant(P<0.05)Compared with the three treatment groups of neuromobilization,the SFI of 9%of neuromobilization was higher than that of the other two groups,the difference was statistically significant(P<0.05),the 6%of neuromobilization was higher than the 12%of neuromobilization,the difference was not statistically significant(P>0.05).Except for the normal group,the SFI of each group peaked at 4 weeks4.Corresponding sciatic nerve and spinal ganglion have myeloid axon count:after 1,2,and 4 weeks of treatment,the sciatic nerve and spinal ganglion medullary axon counts of the three neuromobilization groups are higher than the model group and lower than the normal group.Statistical significance(P<0.05).Comparing the three treatment groups of neuromobilization,the sciatic nerve and spinal ganglion had axon counts of sciatic nerve and spinal ganglion in 9%of the groups,and the difference was statistically significant(P<0.05).Except for the normal group,the counts of myelinated axons of the sciatic nerve and spinal ganglion in each group reached their peak at 4 weeks5.After 1 week of treatment,the expression of NGF in the 6%of neuromobilization group,the 9%of neuromobilization group,and the 12%of neuromobilization group was higher than that in the normal group,and the difference was statistically significant(P<0.05);The expression of NGF in the 6%of neuromobilization group,the 9%of neuromobilization group was higher than that of model group,the difference was statistically significant(P<0.05),the expression of NGF in 12%of neuromobilization group was higher than that of model group,and the difference was not statistically significant(P>0.05).After 2 weeks and 4 weeks of treatment,the expression of NGF in the model group was higher than that in the normal group,and the difference was statistically significant(P<0.05);the expression of NGF in the 6%neuroleptic group,9%neuroleptic group,and 12%neuroleptic group Higher than the model group and normal group,the difference was statistically significant(P<0.05).Compared with the three treatment groups of neuromobilization,the expression of NGF in 9%of neuromobilization group was higher than that of 6%of neuromobilization group and 12%of neuromobilization group,the difference was statistically significant(P<0.05),and 6%of neuromobilization group It was higher than the 12%group of neuromobi-lization,and the difference was not statistically significant(P>0.05).In 3 times,NGF expression peaked at the 4 week time point Except for the normal group,the expression of NGF in each group reached its peak in 4 weeksConclusions1.In this experiment,the optimal neuromobilization parameter for rat sciatic nerve mobilization was 9%.2.Nerve mobilization within the range of effective neuromobilization parameters(tensile parameters 6%,9%,12%)can increase the motor nerve conduction velocity,sciatic nerve function index and corresponding sciatic nerve and spinal ganglion myelinated axon counts in rats with sciatic nerve injury,and can promote the expression of NGF protein,the best curative effect of nerve mobilization surgery is 9%3.Promoting axonal regeneration after sciatic nerve injury by nerve mobilization within the range of effective stretching parameters may be related to increased NGF protein expression.
