| Objective Through the stimulation of cerebellar fastigial nucleus with biomimetic electrical stimulator to treat patients with cerebral infarction,observe its clinical effect on cognitive dysfunction after cerebral infarction,study new ideas of treating cognitive impairment,and explore its possible mechanism,so as to provide clinical basis for experimental research and rehabilitation treatment.Methods From January 2018 to July 2019,50 patients with cognitive impairment after cerebral infarction were selected from the Department of rehabilitation medicine of our hospital,and all of them had hemiplegia of one side of limbs.They were randomly divided into treatment group and control group,25 cases in each group.The patients in the two groups received drug intervention and routine rehabilitation treatment,including cognitive function training,hyperbaric oxygen therapy,occupational therapy,exercise function training,balance function training and other methods,while in the treatment group,EEG biomimetic electrical stimulator was used to stimulate the fastigial nucleus.Patients in the two groups were assessed with the Montreal Cognitive Assessment(Mo CA),Mini Mental State Examination(MMSE),NIHSS and modified Barthel Index(MBI)before and after treatment.Among them,MOCA and MMSE were used to evaluate the cognitive function,NIHSS was used to evaluate the neurological deficit symptoms,MBI was used to evaluate the activities of daily living(ADL)ability,and transcranial Doppler ultrasound(TCD)was used to evaluate the hemodynamic changes of intracranial arteries.Results1,Cognitive function score: after treatment,MOCA score of the treatment group and the control group were(25.48 ± 2.35)and(20.32 ± 3.08)respectively,MMSE score was(28.24 ± 2.01)and(25.12 ± 2.28)respectively,both increased compared with before treatment(P < 0.05),and cognitive function of the treatment group was improved more significantly than that of the control group(P < 0.05).2,Neurological deficit score: after treatment,NIHSS scores of the treatment group and the control group were(12.52 ±6.05)and(18.16 ± 6.53)respectively,which were lower than those before treatment(P< 0.05),and the recovery of neurological function in the treatment group was more significant(P < 0.05).3,ADL ability score: the MBI scores of the patients in the treatment group and the control group were(73.40 ± 9.70)and(55.80 ± 9.10)respectively,which were higher than those before treatment(P < 0.05),and the ADL ability of the treatment group was improved more significantly(P < 0.05).4,Hemodynamic evaluation: the hemodynamics of intracranial arteries in the two groups were improved after treatment(P < 0.05),and the improvement in the treatment group was more significant(P < 0.05).5,Total effective rate: the total effective rate of the treatment group and the control group after treatment was 92% and 64%,respectively,the difference was statistically significant(P < 0.05).Conclusion At the same time of drug intervention and routine rehabilitation treatment,stimulation of fastigial nucleus with EEG biomimetic electrical stimulator can increase blood flow of local and cerebral cortex,improve cerebral perfusion and cerebral circulation,effectively improve cognitive function of patients with cerebral infarction,protect nerve cells,accelerate recovery of nerve function,improve symptoms of nerve function defect,improve quality of daily life,and improve rehabilitation Therapeutic effect.Its possible mechanism of action is to further improve the cognitive function and neural function by improving the cerebral circulation of patients,which is safe,effective and easy to operate.This therapy is suitable for clinical implementation and application. |
