| Background:Coronary atherosclerotic heart disease refers to a type of heart disease,coronary heart disease for short,which is caused by atherosclerotic lesions leading to stenosis or occlusion of the coronary artery lumen and eventually leads to ischemia?hypoxia and even necrosis of the myocardium supplied by the corresponding arteries.The organ most frequently involved in atherosclerosis is the heart,that will do serious harm to the health of people.Percutaneous coronary intervention is a group of percutaneous coronary intervention techniques that include Percutaneous Transluminal Coronary Angioplasty?Atheromatous plaque ablation technique and Coronary artery stent implantation techniques.Since the first application of PTCA in clinical practice in 1977,PCI has become an important treatment in the field of coronary heart disease.Multiple clinical trials and studies have suggested that compared with simple conservative medical treatment,PCI can significantly improve the treatment effect and prognosis of coronary heart disease patients.After many years of technical development and clinical research,coronary stent technology has been developed and innovated continuously.The earliest bare metal stents have been eliminated in clinical treatment due to the high restenosis rate after implantation,and drug-eluting stents have come into being subsequently.In a series of clinical studies,it has been proved that the treatment effect is ideal and the safety is certain.Therefore,PCI gradually accepted by patients and their families because of its advantages such as small trauma and remarkable treatment effect,but we frequently encountered the question that which type of stents they should choose in this operation,Therefore,this research will use the medical histories of those patients who were diagnosed as coronary heart disease,underwent PCI and used domestic or imported stents respectively in our hospital,to provide true reference for clinical work.Objective:In the department of cardiology of our hospital,the efficacy and safety of the domestic Firebird2sirolimus-eluting stents and the imported Endeavor Resolute zotarolimus-eluting stents were compared among patients who were clearly diagnosed as coronary heart disease and treated with PCI,providing real-world evidence for the selection and optimization of coronary stents.Methods:Using the historical cohort study in this research,participants included 353 patients who underwent PCI treatment in patients with coronary heart disease,according to the type of stent was divided into Firebird2 group(188 cases)and Endeavor Resolute group(165 cases).Gathering the baseline information and the operation data of these patients,observing the treatment effect of two groups of stents,follow-up for 12 monthsafter the operation.Considering the major adverse cardiovascular events(MACE)as the endpoint of the observation,which include re-hospitalization(recurrent angina),heart failure(HF),target lesion revascularization(TLR),acute myocardial infarction(AMI),cardiogenic death,and all-cause death(death of any cause).The data were collected and analyzed by SPSS23.0 software.Results:General clinical data(gender,smoking history,hypertension,dyslipidemia,diabetes)of the two groups showed no significant difference(P>0.05).The chi-square test and Fisher's exact probability method of the number of implanted stents showed no statistical significance,and the t-test of the mean number of implanted stents showed no statistical significance(P>0.05)too.No statistical significance was found in chi-square test of admitting diagnosis and vascular lesions(P>0.05);Follow-up result: 38 patients in the Firebird2 stents group and 16 patients in the Endeavor Resolute stents group were excluded according to the exclusion criteria,and the final data of two sets were 188 and 165 respectively.There were 41 and 40 patients were diagnosed with recurrent angina respectively;The cases of acute myocardial infarction were 1 and 0 respectively;The cases of severe arrhythmia were 5 cases and 3 cases;there were 1 and 0 cases of cardiogenic death respectively.Heart failure was 6 and 4 cases;Target lesionl revascularization was performed in 3 patients and 1 patients;The cases of in-stent restenosis were 4 and 3 respectively.The chi-square test and Fisher's exact probability were used to test the specific MACE events in the two groups,and there was no significant differencein the comparison(P>0.05).Conclusion : There was no statistical difference in the clinical efficacybetween Firebird2? sirolimus-eluting stents and the imported Endeavor Resolute zotarolimus-eluting stents. |
