Application Study Of Functional Exercise Intervention Programs Based On PRECEDE-PROCEED Model In Patients With Tibial Plateau Fracture

Abstract/Summary:
ObjectiveTo formulate and implement intervention programs for functional exercise of tibial plateau fracture(TPF)patients using the PRECEDE-PROCEED model.To verify the effects of the application of functional exercise nursing intervention program based on the PRECEDE-PROCEED model on knee joint function,functional exercise compliance,and functional exercise satisfaction in patients with tibial plateau fractures.MethodsThis study was designed using quasi-experiment.Patients with tibial plateau fractures treated in the trauma surgery department of a tertiary comprehensive hospital in Qingdao from July 2018 to August 2019 were selected as the research objects.With the convenient sampling method,46 patients who were hospitalized in the second ward of trauma surgery and met the inclusion criteria were set as the control group,46 patients who met the inclusion criteria in the first ward of trauma surgery were set as the observation group.The control group implemented routine nursing.For the observation group,we evaluated and diagnosed the influencing factors of functional exercise in patients with tibial plateau fractures,formulated and implemented corresponding nursing interventions based on the PRECEDE-PROCEED model.And then analyzed and compared the knee joint function scores,functional exercise compliance,and functional exercise satisfaction of the two groups at different time points,such as before discharge,1week after surgery,1 month after surgery,3 months after surgery,and 6 months after surgery.The differences between them were used to evaluate the effect of functional exercise intervention programs based on the PRECEDE-PROCEED model in patients with tibial plateau fractures.Result1.Comparison of HSS(Hospital for Special Surgery)knee function scores between the two groups: there was no statistical difference between the two groups of patients before intervention(P>0.05);Compared with the HSS knee function scores at 1 month after surgery,3 months after surgery,and 6 months after surgery,the observation group was higher than the control group(P<0.05).The HSS knee function scores of both groups of patients showed an upward trend with time,and the increase in the observation group was larger than that of the control group,indicating that the knee function of theobservation group was better than that of the control group.And the speed of knee function recovery was relatively faster.2.Comparison of functional exercise compliance between the two groups of patients:the functional exercise compliance scores at 1 week after surgery,1 month after surgery,3 months after surgery and 6 months after surgery were significantly higher in the observation group than in the control group.And the difference was significant statistically(P<0.05).The comparison of functional exercise compliance within the group showed that the change in functional exercise compliance of each group of patients was statistically significant(P<0.05),in which the functional exercise compliance score was significantly higher 1 month after surgery,3 month after surgery,6 month after surgery than 1 week after surgery respectively(P<0.05).The compliance score of the control group decreased from 1 month after surgery to 3 months after surgery(P<0.05),but there was no obvious change in the observation group.And there was no significant change 6months after surgery compared to 3 months after surgery(P>0.05),indicating that the compliance of functional exercise in the observation group was better than the control group.3.Comparison of patient functional exercise satisfaction between the two groups: the satisfaction scores of the observation group were higher significantly than the control group,and the difference was significant statistically(P<0.05).Among them,satisfaction scores of the observation group in five dimensions,such as nurses’ high-efficiency professionalism and trustworthiness,functional exercise guidance and evaluation,pain and psychological intervention,family training,home exercise guidance and follow-up,were significantly higher than those of the control group,and the differences were statistically significant(P<0.05),indicating that the functional exercise satisfaction of observation group was higher than control group.ConclusionBased on the PRECEDE-PROCEED model,a scientific and systematic tibial plateau fracture functional exercise intervention program was established and applied to patients with tibial plateau fractures,which can significantly improve patients’ knee joint function,functional exercise compliance and satisfaction,and effectively improve functional exercise effects and patients’ quality of life.This model and plan can be applied to clinical nursing practice in trauma orthopedics,providing reference and guidance for nursing work.
