Purposes:At present,the incidence of diabetes mellitus is rising rapidly in China.Diabetic complications are the main factors affecting the prognosis of diabetic patients,and diabetic nephropathy is one of the important factors.At present,the main diagnostic criteria of diabetic nephropathy are urinary albumin excretion rate and 24-hour urinary protein quantification.However,patients' urinary protein may be affected by many factors,and the sample collection and testing methods are cumbersome.Therefore,it is particularly important to find a simple and effective early diagnosis index for improving the early diagnosis rate of diabetic nephropathy.Many studies at home and abroad have shown that the level of lipoprotein(a)is closely related to the degree of diabetic nephropathy.The purpose of this study was to investigate the relationship between lipoprotein(a)and diabetic nephropathy,and to further explore the possible mechanism of the effect of lipoprotein(a)on diabetic nephropathy by measuring the level of CD62 p.Methods:The subjects were 144 diabetic patients admitted to the Department of Endocrinology,Yan Tai Yu Huang Ding Hospital,from December 2017 to December2018.Among them,69 were female and 75 were male,aged between 32 and 85.According to the urinary albumin excretion rate,the subjects were divided into group A diabetes mellitus group(48 cases,25 males and 23 females,urinary albumin excretion rate < 20ug/min),group B early diabetic nephropathy group(48 cases,25 males and 23 females,20ug/min ? urinary albumin excretion rate < 200ug/min),group C clinical diabetic nephropathy group(48 cases,25 males and 23 females,albumin excretion rate ?200 ug/min).The basic information of the three groups were collected,such as age,sex,height,weight,systolic blood pressure,diastolic blood pressure,course of diabetes,smoking history,drinking history,etc.Biochemical indicators of patients were collected:glycosylated hemoglobin,fasting C-peptide,triglyceride,total cholesterol,high density lipoprotein,low density lipoprotein,lipoprotein(a),total bilirubin,direct bilirubin,indirect bilirubin,alkaline phosphatase,gamma-glutamyltransferase,alanine transaminase,glutathione transaminase,urea,creatinine and uric acid.CD62 p was measured in three groups.Results:144 cases were included in this study,including 48 cases of simple diabetes mellitus:urinary albumin excretion rate < 20ug/min,30 males,18 females,age 57(52,67);48cases of early diabetic nephropathy group: 20ug/min ? urinary albumin excretion rate <200ug/min,30 males,18 females,age 64(54.75,72);48 cases of clinical diabetic nephropathy group: urinary albumin excretion rate ? 200ug/mi N,30 males and 18 females,aged 63.5(54.5,71).A total of 144 patients completed all assessments.Results:1.lipoprotein(a)in early diabetic nephropathy group was significantly higher than that in simple diabetes group,and lipoprotein(a)in clinical diabetic nephropathy group was significantly higher than that in early diabetic nephropathy group.2.Lipoprotein(a)is an independent risk factor for the occurrence and development of diabetic nephropathy.3.CD62 p in early diabetic nephropathy group was significantly higher than that in simple diabetic nephropathy group,and CD62 p in clinical diabetic nephropathy group was significantly higher than that in early diabetic nephropathy group.4.Lipoprotein(a)was positively correlated with CD62 p in patients with diabetic nephropathy.Conclusions:1.This study indicated that lipoprotein(a)is associated with the degree of renal damage in diabetic nephropathy,and may be one of the risk factors for the occurrence and development of diabetic nephropathy.Because of its simple detection method,it can be used as one of the auxiliary means in the diagnosis of diabetic nephropathy.2.This study indicated that lipoprotein(a)is positively correlated with CD62 p in patients with diabetic nephropathy.Therefore,we speculate that lipoprotein(a)may promote the development of diabetic nephropathy mainly by promoting platelet activation.
