Objective: The level of serum lipoprotein(a) [Lp(a)],Von willabrand factor(vWF),D-Dimer was measured by ELISA both in normal controls and cases with type 2 Diabetes Mellitus(T2DM),so did the Fg,APTT,PT,TT detected by an automatic coagulometer autoanalyzer in groups. The levels of fasting blood glucose(FBG) ,post blood glucose(PBG) ,hemoglobinA1c(HbA1c),systolic blood pressure(SBP),diastolic blood pressure(DBP), plasma lipid includes total cholesterol(TC) ,total triglyceride(TG) ,high-density lipoprotein(HDL),low-density lipoprotein(LDL),apoA,apoB, blood ureanitrogen(BUN),blood creatinine(Cr) et al were also measured. T2DM patients were divided into three groups based on the 24 hours urine albumin excretion rate(UAER), including without albuminuria(A group) ,with microalbuminuria(B group),with macroalbuminuria(C group). Compared with those parameters in above groups and found the differences among above groups. Furthermore, we also investigated the relationship among those parameters and the possible correlation between Lp(a) and every one of vWF,D-Dimer,Fg,UAER,BUN,so we could realize the metabolic abnormality state of plasma lipiprotein and blood glucose and the changes of plasma haemostatic and fibrinolytic activities in T2DM patients; we study the changes of Lp(a),plasma haemostatic and fibrinolytic activities and their effects on the onset and development of diabetic microangiopathy—diabetic nephropathy(DN) in order to study the significance of the pathogenesis of DN, So that we mey be offered some direction to decelerate the development of DN and some useful methods and some grounds to prevent and monitor DN. Methods: we use the case-control study. 71 patients with T2 DM (1999 WHO diagnostic standards, 35 male and 36 female, 59.4±8.0 years old) are studied. None had present acute or chronic inflammatory disease history of liver or kidney disease, acute complication of DM, for example diabetic ketoacidosis(DKA) ,nonketotic hyperosmolar diabetic coma(NHDC), heart failure or other endocrinopathy diseases,for example hyperthyroidism or hypothyroidism,hyperuricemia, pernicious tumor, haemal disease, or use medicine which effects on kidney function , using anticoagulation or cholesterol-lowering medicine in last two weeks. DM patients were divided into three groups based on 24hUAER measurements, including 25 cases without albuminuria(A group), 12 male and 13 femal, 20 cases with microalbumiuria(B group), 9 male and 11 femal, 26 cases with macroalbumiuria(C group), 14 male and 12 femal. And taken 18 healthy persons as normal controls,10 male and 8femal, 57.1±8.3 years old. After an overnight fast(>10h), venous blood samples were obtained in the next morning and divided into 2 parts: serum was selected from one part without anticoagulation fluid and measured the level of Lp(a) by ELISA; plasma was selected from the other with anticoagulation fluid and measured the levels of plasma vWF,D-Dimer,Fg by two-site sandwich enzyme-linked immunosorbent assay(ELISA). The levels of SBP,DBP,HbA1c,BMI,FBG,PBG,BUN,Cr,APTT,PT,TT,plasma lipids et al were also measured, meanwhile, the urine samples of 24 hours were taken for measuring albuminuria to calculate UAER. We compared these data in above groups to investigate the relationship among Lp(a) ,D-Dimer,vWF and the possible correlation between every one of them and UAER,BUN,Cr et al,we also took the Lp(a),D-Dimer,vWF and Fg as dependent variables, and took the BMI,HbA1c ,SBP,DBP,FBG,PBG,age,DM duration as independent variables to study the relationship or correlation among them. We use Logistic regression in order to evaluate the dangerous factors of DN. All the data were expressed as mean ±SE. Statistical comparisons were done using independent-sample T test or one-way ANOVA. Relations of variables were described using linear correlation and multiple stepwise logistic regressive analyses.
