Research On Family Doctor Policy Implementation Effect And Improvement Countermeasures Based On Smith Model

General Secretary Xi Jinping clearly mentioned in the report of the Nineteenth National Congress of the Communist Party of China: "People's health is an important symbol of national prosperity and national prosperity.It is necessary to improve the national health policy and provide the people with a full range of full-cycle health services...It is necessary to strengthen the primary health care service system and the construction of the team of general practitioners," and strengthening the primary health care service system and the construction of the team of general practitioners is the necessary means to promote the implementation of the family doctor policy.In2016,the country issued the "Outline of " Healthy China 2030 " Plan" and "Guidelines on Promoting Signing Services of Family Doctors",which marked the formal proposal and full implementation of the family doctor policy.Judging from the current state of the national implementation of the family doctor system,the family doctor policy has had a positive impact on promoting the distribution of medical and health resources,improving medical service capabilities and quality,and controlling residents' medical expenses.Service guarantee capability is the basis for achieving universal health.However,from the current reality,China's primary medical and health service system is hollow,and the level of medical and health services varies greatly among regions.During the implementation of the family doctor policy,there is a low level of awareness of the family doctor policy.The practical problem of low service rate of family doctors' effective contract and uneven distribution of human resources of family doctors.Therefore,the family doctor policy implementation effect is not good,the policy recognition and participation enthusiasm of residents and medical staff are generally low,and government departments and primary medical service institutions have not established a sense of trust between residents and family doctors.Based on the research results of family doctor policy at home and abroad,this paper selects city H as the main research object based on the fact that there are many poor counties in city H,family doctor policy coverage is wider,and policy implementation is more in-depth.In this study,through the interview method,questionnaire survey method,and with the help of existing policy documents and first-hand data from relevant departments,the author learned that City H has made achievements in the standardization of grass-roots medical service institutions,the construction of the family doctor talent team,and the family doctor policy promotion.Certain effects,but from the results of interviews and questionnaires,City H is still relatively weak in the recognition of family doctor policy and service utilization of family doctor policy.Therefore,based on the current status of the implementation of the family doctor policy in City H,according to the problems manifested in the policy implementation process,the Smith policy implementation process model is used to implement the family doctor policy implementation from four aspects of the policy itself,the implementing agency,the target group and the policy environment.Analyze the influencing factors,analyze the cause of the problem,and propose targeted improvement measures.The study found that the reasons for the poor implementation of family doctor policy in H city are mainly manifested in the unreasonable design of the policy text,the lack of ability of the implementing agency,the weak policy recognition of the target group,and the insufficient support of the policy environment.Finally,from the perspective of influencing factors,the countermeasures for improving the implementation of family doctor policy in H city are proposed,mainly from improving the top-level design of the policy,expanding the supply of family doctor policy implementation resources,enhancing the family doctor policy acceptance of the target group,and creating a good family doctor policy Execution environment to elaborate separately.
