Research On Problems And Countermeasures Of Family Doctor Policy Implementation In Guangxi Based On Smith Model

In June 2016,the state began to implement the family doctor policy in an all-round way,set up the first-visit system at the grass-roots level,and set up a team with family doctors as the core to provide contractual basic public health services and basic medical services for residents.Family doctor policy is an important part of Chinese medical system reform.Whether it can be implemented smoothly is related to the vital interests of the people of the whole country,and the social attention is high.Effective implementation of family doctor policy is the key to establish a hierarchical diagnosis and treatment system and to build a reasonable order of social medical treatment.Based on the Smith model,this paper studies the implementation of family doctor policy in Guangxi Zhuang Autonomous Region(hereinafter referred to as"Guangxi”)from four factors:policy text,policy implementation agencies,target groups and policy environment.Based on the systematically combing the relevant literature,through literature research,on-site interviews and questionnaires,this paper summarizes and analyses the current situation of the implementation of family doctor policy in Guangxi,explores the problems encountered by grass-roots medical and health institutions in the implementation of this policy,and finally puts forward the countermeasures to solve the problems.The study found that the main restrictive factors for the implementation of family doctor policy in Guangxi are:unreasonable and unclear policy itself,imperfect policy supervision and assessment system,the level of executing agencies and staffing needs to be improved,the ability of executing personnel is insufficient,and the information system construction is imperfect;the recognition of target group policy is not strong,and the benefit of policy to target group is not obvious;Laws are not perfect,financial support is not strong,and social attention is insufficient.On this basis,this paper puts forward appropriate countermeasures to solve these problems,including fourth aspects:first,improve the top-level design of family doctor policy,improve the rationality,legitimacy of policy and the feasibility of achieving goals;secondly,strengthen the capacity-building of executive agencies and enhance the professional ability of family doctor,improve policy information system,and improve the effectiveness of policy implementation.The third is to enhance the awareness and recognition of the family doctor policy by the target group through strengthening publicity,and finally to improve the political,economic and social environment related to the implementation of the policy.
