Discussion On The Medication Rule Of Professor Mi Ziliang In The Treatment Of Distention And Fullness Based On The Traditional Chinese Medicine Inheritance Support System

Objective Based on the Traditional Chinese Medicine Inheritance Support System,a study of TCM master Mi Ziliang’s medication rule in prescriptions for distention and fullness.It is summarize and inherit Professor Mi Ziliang’s experience in the diagnosis and treatment of distention and fullness,and to provide new ideas and contents for the clinical application and scientific research of traditional Chinese medicine in treating distention and fullness.Methods Collect and sort out the medical records of the treatment of distention and fullness in the outpatient department of Mongolian Chinese Medicine Hospital affiliated to Inner Mongolia Medical University and the Chinese Medical Hospital of Inner Mongolia Autonomous Region from January 2016 to December 2018.Build database,Use Traditional Chinese Medicine Inheritance Support System,Data mining methods such as improved mutual information method,complex system entropy clustering method,unsupervised hierarchical clustering analysis,etc.are analyzed by counting the frequency of drug use,the four nature of drugs,the five flavors,the channel entry,and the association rules between drugs.Summarize the medication rules and prescription characteristics of Professor Mi Ziliang in the diagnosis and treatment of distention and fullness.Results A total of 652 distention and fullness cases were collected in this study.By analyzing basic data,the patient population between the ages of 40 and 69 accounted for 66.10% of the total number.Female patients accounted for 58.44% of the total number.Analysis of the main symptoms of distention and fullness patients,there are12 symptoms more than 200 times,distention and fullness,anorexia,loose stool,acid regurgitation,weakness,belching,Cold stomach,insomnia,hiccup,gastric upset,nausea,vomiting,bitter taste in mouth.Analysis of the tongue manifestation of the patient found that the tongue manifestation of the patient was mainly pink tongue and white coated tongue,and the tongue shape was changed,with fissured tongue or depressions in the tongue manifestation being the most prominent.Analysis of the patient’s pulse manifestation found that the patients were mainly concurrent pulse,most of which were deep pulse,stringy pulse,thready pulse,faint pulse,moderate pulse,and rapid pulse.The pulse manifestation characteristics are mainly manifested by the abnormalities in the right part.distention and fullness disease is mainly due to spleen and stomach deficiency syndrome,followed by other syndromes such as mixed cold and heat and insufficient stomach yin etc.There are 177 of Chinese medicine in this study,34 of Chinese medicines used more than 100 times,Among them,the traditional Chinese medicines used more frequently are medicated leaven,hawthorn fruit,germinated barley,officinal magnolia bark,pinellia tuber,inula flower,heterophylly falsestarwort root,clam shell,largehead atractylodes rhizome,Indian bread,dried tangerine peel,inner membrane of chicken gizzard,orange fruit,tuber fleeceflower stem,radish seed,spine date seed,golden thread,grassleaf sweetflag rhizome,Chinese angelica,weeping forsythia capsule,baical skullcap root,dwarf lilyturf tuber,etc.Divided according to the nature of Chinese medicine,In terms of "four nature of drugs",warm drugs and flat drugs accounted for the largest proportion.In terms of "Five Flavors",the medicines are mainly bitter,pungent and sweet.In terms of "channel entry",the four meridians of the Taiyin Spleen Channel of Foot,Yangming Stomach Channel of Foot,Taiyin Lung Channel of Hand and Jueyin Liver Channel of Foot had the highest frequency.Based on the improved mutual information method and the entropy clustering of complex systems,discovered 26 drug combinations.for example,pinellia tuber-officinal magnolia bark-medicated leaven-hawthorn fruit-germinated barley combination,pinellia tuber-officinal magnolia bark-inula flower-medicated leaven-hawthorn fruit-germinated barley combination,officinal magnolia bark-inula flower-orange fruit-areca seed combination,pinellia tuber-Indian bread-dried tangerine peel-tangshen and other 26 drug combinations.Based on the data algorithms built into the platform,eight newprescriptions were proposed for the treatment distention and fullness.New prescription(1),(pinellia tuber,officinal magnolia bark,inula flower,medicated leaven,hawthorn fruit,germinated barley,clam shell,heterophylly falsestarwort root).New prescription(2),(pinellia tuber,inula flower,medicated leaven,hawthorn fruit,germinated barley,daizheshi,areca seed,rhubarb root and rhizome).New prescription(3),(pinellia tuber,officinal magnolia bark,medicated leaven,hawthorn fruit,germinated barley,largehead atractylodes rhizome,dried tangerine peel,orange fruit,finger citron,common aucklandia root,areca seed).New prescription(4),(medicated leaven,hawthorn fruit,germinated barley,heterophylly falsestarwort root,inner membrane of chicken gizzard,liquorice root,common yam rhizome).New prescription(5),(pinellia tuber,inula flower,heterophylly falsestarwort root,inner membrane of chicken gizzard,golden thread,baical skullcap root,zedoray rhizome,common bletilla rubber,zingiber).New prescription(6),(pinellia tuber,officinal magnolia bark,inula flower,dried tangerine peel,orange fruit,golden thread,zingiber,snakegourd fruit).New prescription(7),(dwarf lilyturf tuber,dendrobium,fragrant solomonseal rhizome,unprocessed rehmannia root,fourleaf ladybell root,common yam rhizome,snakegourd root).New prescription(8),(lesser galangal rhizome,nutgrass galingale rhizome,common yam rhizome,cassia twig,epimedium herb).Conclusions Professor Mi Ziliang believes that the weakness of the spleen and stomach and the qi movement incoordination are the underlying causes of distention and fullness.It is considered that tongue manifestation and pulse manifestation are very important for the diagnosis of distention and fullness.In the pattern of distribution of syndrome types,three syndrome types are common: weak spleen and stomach syndrome,mixed syndromes of cold and heat,and insufficient stomach-yin syndrome.Attaching importance to the theory of "free movement of the six fu viscera",protecting the stomach qi,and attaching importance to the physiological functions of the stomach.In terms of treatment,it is biased to support fuzheng,attach importance to strengthening the stomach and nourishing the spleen,regulating the qi movement,and regulating the yin and yang.In terms of drug selection,tonic medicines are mainly used,supplemented with Xiaoshi,Qingre,Huatan and Li Qidrug.In terms of medicinal properties,warm and flat drugs are mainly used,and the three main types are bitter,pungent,and sweet.Frequently used drugs such as medicated leaven,hawthorn fruit,germinated barley,officinal magnolia bark,pinellia tuber,inula flower,heterophylly falsestarwort root etc.Through unsupervised entropy cluster analysis,26 groups of drug combinations for distention and fullness and 8 new prescriptions for distention and fullness were selected.Generated new prescriptions such as pinellia tuber,officinal magnolia bark,inula flower,medicated leaven,hawthorn fruit,germinated barley,clam shell,heterophylly falsestarwort root.All have the effects of strengthening the spleen and stomach,regulating the qi movement,and eliminating distention and fullness.Clinically,they can be used as the basis for treatingdistention and fullness according to the new prescription,and can be added or subtracted according to different conditions.
