Objectives To investigate the correlation between thioredoxin-interacting protein(TXNIP)and peripheral nerve conduction velocity(NCV)in type 2 diabetes mellitus(T2DM).Methods A total of 338 T2DM patients who were treated in Hebei General Hospcital from December 2018 to August 2019 were selected,the basic information and serological indicators of the subjects were collected.The concentrations of TXNIP and tumor necrosis factor-?(TNF-?)were detected by ELISA.The serum glutathione(GSH),superoxide dismutase(SOD),and malondialdehyde(MDA)concentrations were determined by enzyme-labeled immunobiochemical method.Dantec keypoint instrument was used to measure bilateral ulnar nerve movement,ulnar nerve sensation,median nerve movement,median nerve sensation,common peroneal nerve movement and superficial peroneal nerve sensory conduction velocity of patients.According to their peripheral nerve conduction velocity anomaly can be divided into normal group(NCVN group)nerve conduction(183cases,mean age 53.62±11.53 years old,with 126 cases male patients,female patients,57cases)and abnormal nerve conduction(NCVA group)(155 cases,with an average age of58.96±12.47 years,with 97 cases male patients,58 patients with female).SPSS 21.0 was used for statistics,and the significance level was all P<0.05.Results 1 Comparison of indexes between normal group and abnormal group of peripheral nerve conduction velocity in T2DM: Compared with the NCVN group,patients in the NCVA group were older,had a long course of diabetes,had high levels of fasting blood glucose,glycated hemoglobin,and HOMA-IR,standard deviation of blood glucose(SDBG),lage blood glucose fluctuation(LAGE),and mean blood glucose(MBG)are increased,and had high levels of TXNIP and MDA(P<0.05).Compared with the NCVN group,patients in the NCVA group had lower diastolic blood pressure,vitamin D3 level,serum albumin level,alanine aminotransferase and glomerular filtering rate,and there were statistical differences in the use of insulin and metformin(P<0.05).There were no significant differences in gender,body mass index,systolic blood pressure,total cholesterol,triglyceride,high density lipoprotein,low density lipoprotein,glutamic oxaloacetate transaminase,creatinine,urea nitrogen,uric acid,total triiodothyronine,total thyroxine,thyroid stimulating hormone,postprandial blood glucose fluctuation(PPGE),GSH,TNF-? and SOD between the two groups(P>0.05).2 Correlation between peripheral nerve conduction velocity and serum TXNIP in T2DM patients: In T2DM patients,logistic regression analysis showed that age,fasting blood glucose,SDBG,TXNIP were risk factors for abnormal NCV,while vitamin D3 were protective factors for abnormal NCV.After adjusting for age,course of diabetes,fasting blood glucose,glycosylated hemoglobin,vitamin D3,SDBG,LAGE and MBG,TXNIP was still an independent risk factor for NCV.3 Effect of TXNIP on peripheral nerve conduction velocity in type 2 diabetes mellitus: It was found that TXNIP was an independent risk factor for left ulnar motor conduction velocity(MCV),right ulnar MCV,left median MCV,and right median MCV.4Correlation between TXNIP and oxidative stress and inflammation index: Taking serum TXNIP as the dependent variable,age,course of disease,fasting blood glucose,glycosylated hemoglobin,HOMA-IR,vitamin D,SOD,SDBG,TNF-?,GSH,MDA,use of insulin,use of metformin as the independent variable,After multivariate stepwise regression,it was found that TXNIP was the positive influencing factor of serum TXNIP(P<0.05).Meanwhile,serum GSH,MDA,SOD and TNF-? were used as dependent variables,and age,course of disease,fasting blood glucose,glycosylated hemoglobin,HOMA-IR,vitamin D3,SDBG,TXNIP,use of insulin,use of metformin as independent variables.After multivariate stepwise regression,serum TXNIP was a positive factor of MDA and TNF-? in T2DM(P<0.05).Conclusions 1 In T2DM patients,the level of TXNIP and MDA in NCVA group increased.2 In T2DM patients,age,fasting blood glucose,SDBG and TXNIP are the risk factors for abnormal NCV,and vitamin D3 is the protective factor for abnormal NCV.TXNIP has a close relationship with nerve conduction velocity of both upper limbs.3 In NCVA group,TNF-? is a positive factor of TXNIP.At the same time,TXNIP can significantly affect the levels of TNF-? and MDA.TXNIP may interact with oxidative stress and inflammation,leading to the occurrence of DPN.Figure [1];Table [15];Reference [159].
