Objective:Based on clinical data of patients with type 2 diabetes,collect and analysis of TCM syndrome type and type 2 diabetes and insulin resistance in sarcopenia correlation research.And analysis the age,sex,duration of diabetes,insulin sensitive index,insulin resistance,homeostasis model assessment-?and sarcopenia.And discussing the different TCM syndrome types and insulin resistance with sarcopenia occurrence and development of possible relations.Exploring the effective prevention and cure of sarcopenia and insulin resistance.Methods:The 300 patients who were admitted to the affiliated hospital of Cheng Du University of TCM,from January 2018 to January 2019.And met the diagnosis of type2 diabetes mellitus were selected to collect basic information such as age,medication history,family history,allergy history,and course of diabetes.General indexes such as height,weight,waist circumference,were measured.And Body Mass Index(BMI)was calculated.Liver function:ALT,AST,GGT;Kidney function:BUN,Crea,UA;Lipid profile:TCH,TG,HDL-c,LDL-c;Oral Glucose Tolerance Test(OGTT):FPG,1h GLU,2h GLU,3h GLU;Insulin release assay:FINS,1h INS,2h INS,3h INS;24-hour urinary creatinine,Bone Mineral Density(BMD).And calculating insulin resistance index(HOMA-IR),insulin sensitivity index(ISI),homeostasis model assessment-?(HOMA-?),skeletal muscle index(SMI).Result:In the statistics of 294 cases of T2DM patients,according to the different categories of gender,such as age,medical history,family history of T2DM,quality of T2DM duration,body weight index(BMI),TCM syndrome type groups,can be intuitively seen the the distribution btween patients with sarcopenia and patients with type 2 diabetes is roughly same,can be thought of patients with type 2 diabetes are more likely to suffer from sarcopenia.According to the diagnostic criteria of sarcopenia,patients with type 2 diabetes were divided into sarcopenia group and non-sarcopenia group,and the proportion of the number of patients was compared between the two groups according to gender,age,medication history,family history of T2DM,course of T2DM,body mass index(BMI)and TCM syndrome type.According to gender,age,medication history,course of T2DM,and body mass index(BMI),the difference between the number of patients in the sarcopenia group was compared with that in the non-sarcopenia group was statistically significant(P<0.05).Family history of T2DM and classification of TCM syndromes,the difference between the number of patients in the sarcopenia group and the non-sarcopenia group was compared was not statistically significant(P>0.05).T2DM patients were grouped according to different TCM syndromes.The mean values of FPG,FINS,and homeostasis model assessment-?were all higher in the sarcomere group than in the non-sarcomere group(P<0.05).Insulin sensitivity coefficient(ISI)and insulin resistance index(HOMA-IR)showed no significant difference between sarcopenia and non-sarcopenia in different TCM syndromes(P>0.05).T2DM patients were grouped according to different TCM syndrome types,and there was no significant difference in cholesterol(TCH),triglyceride(TG),high-density lipoprotein cholesterol(HDL-c)and low-density lipoprotein cholesterol(LDL-c)between the sarcopenia group and the non-sarcopenia group(P>0.05),with no statistical significance.The groups were grouped according to the TCM syndrome types,and bone mineral density in the lumbar,Ward and femoral neck.And the difference compared between the bone mineral density in the lumbar and Ward regions and the TCM syndrome types was statistically significant(P<0.05).There was no statistically significant difference between the femoral neck region and TCM syndrome type(P>0.05),and there was no statistically significant difference between the total bone density and TCM syndrome type(P>0.05).The dominant hand grip strength(DHS),daily walking speed(DWS)and calf circumference were compared between the sarcopenia group and the non-sarcopenia group according to gender,respectively.The dominant hand grip strength(DHS)and daily walking speed(DWS)were compared at(P<0.05),with statistically significant differences.The ratio of dominant hand grip strength(DHS)and daily walking speed(DWS)between different TCM syndromes was compared with that between normal and reduced.The dominant hand grip strength(DHS)and daily walking speed(DWS),the difference between different syndrome types was statistically significant(P<0.05).Conclusion:1.Women with type 2 diabetes are more likely to develop sarcopenia than men.2.Type 2 diabetes and myopenia are closely related to age and are age-related diseases.3.Increased progression of type 2 diabetes is an important factor in the development and progression of sarcopenia.4.People with type 2 diabetes who are obese(BMI?30kg/cm~2)are more likely to develop sarcopenia.5.Insulin and metformin,drugs that control blood sugar,are associated with sarcopenia.6.Sarcopenia has no difference among TCM syndromes of type 2 diabetes including phlegm(wet)heat mutual syndrome,heat injury and jin syndrome,qi and Yin deficiency syndrome,liver and kidney Yin deficiency syndrome,Yin and Yang deficiency syndrome.The basic pathogenesis of sarcopenia and type 2 diabetes is the same,that is,"Deficiency of temperament,Loss of movement and transformation".7.There was no difference in lipid metabolism between type 2 diabetic patients with non-sarcopenia and type 2 diabetic patients with sarcopenia.8.Bone mineral density in the lumbar and ward regions of patients with type 2 diabetes differs between patients with non-sarcopenia and patients with sarcopenia, and the bone mineral density in patients with sarcopenia is low.9.In type 2 diabetic patients with sarcopenia,hand grip strength and daily walking speed are decreased,which means muscle strength and exercise ability are decreased in patients with sarcopenia.10.Among different TCM syndromes,there were differences in both hand grip strength and daily walking speed.In other words,it was feasible for 2 diabetic patients to use both hand grip strength and daily walking speed as screening indicators for sarcopenia.
