Objective: Lipoprotein-associated-phospholipase A2(LP-PLA2)is a simple but practical marker of cardiovascular diseases(CVD).A large number of studies have shown that LP-PLA2 is an independent risk factor for cardiovascular diseases.Many reports have reported that LP-PLA2 is an independent risk factor for cardiovascular diseases.Activity was established as an independent predictor of CHD in the general population [1-11].However,there are few studies in the field of diabetes mellitus(DM),and the correlation between diabetes mellitus and glycometabolism is not clear.Therefore,this study intends to analyze the correlation between plasma lipoprotein phospholipase A2(LP-PLA2)level and duration of diabetes mellitus,hemoglobin A1C(HBA1C),insulin resistance index(HOMA-IR)in the form of cross-sectional study,and further study.To explore whether there is correlation between brachial-ankle pulse wave velocity(Ba PWV)and atherosclerosis index in type 2 diabetes mellitus.Methods: 120 diabetic patients admitted to the Endocrinology Department of the First Affiliated Hospital of Nanjing Medical University(Jiangsu People's Hospital)from November 2017 to November 2018 were divided into two groups according to their course of disease: the group under 10 years and the group over 10 years.At the same time,according to the level of glycosylated hemoglobin,the patients were divided into two groups: glycosylated hemoglobin and glycosylated hemoglobin.In the group with white level > 8% and glycosylated hemoglobin < 8%,all patients were examined by physical examination,plasma LP-PLA2,biochemical index,glycosylated hemoglobin,pancreatic islet function test stimulated by steamed bread and Ba PWV.Then we analyzed the relationship between Lp-PLA2 and other indexes in different course groups,and the level of Lp-PLA2 and other indexes in different glycosylated hemoglobin groups.Results: There was no significant difference in general condition(age,sex,bad hobby history)and past chronic disease history among groups,but no significant difference in high density lipoprotein and triglyceride among related examinations(P > 0.05).Data comparison among different course groups: LP-PLA2 level,HOMA-IR level,HBA1 C,Ba PWV level in the course group of less than 10 years were significantly lower than those in the course group of more than 10 years,the difference between the two groups was statistically significant(P < 0.05),while fasting blood glucose was not well controlled because of the selected patients,although there were differences between the two groups,but the difference was poor.There was no significant difference between the two groups(P > 0.05).Similarly,compared with HBA1 C > 8%,the levels of LP-PLA2,HBA1 C,HOMA-IR and Ba PWV were significantly different between the two groups(P < 0.05),except for confounding factors,HOMA-IR and LP-PLA2.Horizontal independent positive correlation(P < 0.001).Conclusions: 1.In patients with type 2 diabetes mellitus,plasma lipoprotein-associated phospholipase A2(LP-PLA2)level is closely related to insulin resistance level,and can be used as an independent risk factor for insulin resistance.Whether LP-PLA2 plays a role in the pathogenesis of insulin resistance needs further study.2.LP-PLA2 is positively correlated with PWV level in diabetic patients,which has clinical value in predicting atherosclerosis and macroangiopathy in diabetic patients.3.LP-PLA2 is positively correlated with glycosylated hemoglobin,which can be used as a screening tool for diabetes control and complications.However,the causal relationship between LP-PLA2 and glycosylated hemoglobin needs further confirmation.4.LP-PLA2 is positively correlated with the course of diabetes mellitus,and gradually increases with the progress of the course,suggesting that LP-PLA2 may be an important reason for the increase of cardiovascular risk in type 2 diabetes mellitus with long course.
