Objective: To evaluate the clinical therapeutic effect of Wenyang Yiqi Huoxue Recipe on chronic heart failure(CHF)by observing the effect of Wenyang Yiqi Huoxue Recipe on clinical symptoms,cardiac function and laboratory test results in patients with CHF,and to preliminarily explore the mechanism of Wenyang Yiqi Huoxue Recipe in treating CHF by observing the effect of Wenyang Yiqi Huoxue Recipe on gene expression profile of peripheral blood nucleated cells in patients with CHF.Methods: Western medicine group,30 cases in each group(that is,60 cases collected by the inpatient department of Qinghai Provincial Hospital of Traditional Chinese Medicine were divided into two groups by random number table).The western medicine group was given conventional western medicine treatment,the course of treatment was 15 days;on the basis of conventional western medicine treatment,Wenyang Yiqi Huoxue Recipe(granules)was added,6 grams/time,twice a day,30 minutes after meals warm,15 days for a course of treatment.Before and after treatment,clinical symptom scores were assessed,cardiac function was measured by echocardiography,and peripheral blood samples were obtained and divided into two parts,one for serum brain natriuretic peptide(BNP)and the other for storage in a-80 ° C refrigerator.Peripheral blood was randomly selected from 10 patients in the two groups before and after treatment to detect differences in nucleated cell gene expression profiles.After a course of treatment,the clinical symptom score,the improvement of cardiac function and the difference of gene expression profile of peripheral blood nucleated cells were observed.Results: On admission,the basic data data of the two groups showed that they were within the normal distribution range,which were not statistically significant by statistical analysis and comparable.However,after a course of treatment after admission,serum brain natriuretic peptide and TCM symptom scores decreased,both groups were effective after treatment,and the difference between the two groups was statistically significant(P < 0.05),indicating that Wenyang Yiqi Huoxue Recipe had the effect of improving clinical symptoms in patients with chronic heart failure;compared with the western medicine group,2396 differentially expressed genes were selected in the traditional Chinese and western medicine group,including 2160 upregulated genes(fold change > 1.5)and 236 down-regulated genes(fold change >-1.5),all P < 0.05;3 genes were up-regulated in western medicine and 6 genes were upregulated in western medicine and down-regulated in western medicine.The upregulated genes were CACNA1 D,CATSPERB and CNBD2,and the down-regulated genes were 8-Sep,CSH2,HSFY2,ORAOV1,SLF2 and ZCCHC9,respectively,indicating that Wenyang Yiqi Huoxue Recipe could regulate the expression of peripheral blood nucleated cell genes in patients with chronic heart failure,and it could be inferred that part of the mechanism of Wenyang Yiqi Huoxue Recipe in the treatment of chronic heart failure was achieved by regulating the expression of genes related to chronic heart failure to improve the pathophysiological function of chronic heart failure.Conclusion: The results showed that Wenyang Yiqi Huoxue Recipe can reduce the clinical symptoms and improve the cardiac function of patients with chronic heart failure,with obvious clinical therapeutic effect.From the results of the study,it can be inferred that part of the mechanism of action of Wenyang Yiqi Huoxue Recipe in the treatment of chronic heart failure is achieved by regulating the expression of genes related to chronic heart failure to improve the pathophysiological function of chronic heart failure.
