Objective: Through the analysis of preoperative core needle biopsy tissues and surgical excision specimens of breast cancer patients,to evaluate the consistency rate of relevant markers(ER,PR,Her2,CK5/6,P53,Ki67,biological classification and pathology classification)by immunohistochemical method.At the same time,the study was to test the CNB tissues to evaluate the sensitivity and specificity of markers(ER,PR,Her2,Ki67)by using the surgical excision specimens as the control.By combining the results of CNB tissues and surgical excision specimens,we can improve the accuracy of diagnosis,and also provide references for patients who need new neoadjuvant treatment and who have no indication of surgery.Methods: I collected clinical and pathological information of 71 breast cancer patients admitted to Qinghai university affiliated hospital from January 2015 to November 2019.CNB used the BARD semi-automatic biopsy gun.Two to four tissues were taken from each breast tumor area.In addition,the pathological sections were reviewed by a pathologist.Results: 1.The coincidence rate of ER and PR staining intensity of core needle biopsy tissues and surgical excision specimens were 75.0% and 71.7%,kappa value was 0.622 and 0.555,P < 0.01.2.The expression of relevant markers(ER,PR,HER2,CK5/6,P53,Ki67)of the CNB tissues and relevant markers of surgical excision specimens in 71 cases breast cancer patients were detected by IHC,the consistent rates were 94.4%,91.5%,84.5%,76.1%,74.6%,78.9%,respectively and kappa values were 0.845,0.802,0.567,0.447,0.487,0.586,respectively.P < 0.01.3.The coincidence rate of molecular typing in CNB tissues and surgical excision specimens was 73.2% and Kappa value was 0.582 in 71 breast cancer patients.P < 0.01.4.The coincidence rate of pathological grading between CNB tissues and surgical excision specimens in breast cancer patients was 64.8% and the Kappa value was 0.414.P < 0.01.5.The sensitivity and specificity of CNB in determining ER were 94.5% and 93.8%,respectively.The sensitivity and specificity of CNB in determining PR were 93.9% and 86.4%.The sensitivity and specificity of HER2 determined by CNB were 87.5% and 73.3%.The sensitivity and specificity of Ki67 determined by CNB were 92.9% and 69.8%.Conclusions: The consistency of ER and PR staining intensity was moderate.Comparison of ER and PR between CNB tissues and surgical excision specimens showed a high consistency of expression in 71 cases of breast cancer patients.The results of Her2,Ki67,P53,CK5/6,biology classification and pathology classification were of moderate consistency.Compared with the surgical excision of the tissue,the sensitivity and specificity of ER by CNB were higher.The results of CNB,ER and PR,can provide reference for neoadjuvant treatment of breast cancer patients and advanced patients.When using CNB results,we need to be cautious.If necessary,combined with surgical excision specimens,we can improve the accuracy of diagnosis,so that patients can receive treatment that can prolonging the overall survival.
