Objective: Breast cancer is hormone-dependent tumors,by estrogen and progesterone regulation, which receptor expression in the majority of breast tissue. In clinical,currently ER, PR expression in breast cancer tissues is to determine whether the applications were able to endocrine therapy. CerbB-2 proto-oncogene expression in breast cancer is related to some clinical pathological features and prognosis,and is a biological target in targeted therapy. Therefore, the mensuration of ER,PR and CerbB-2 expression level in breast cancer,it has an important significance on the choice of treatment for patients and prognostic evaluation. Heterogeneity mainly refers to different tumor cells caused by the difference tumor cell lines in the same tumor tissue [1-4]. Originally, tumor heterogeneity was found in the tissue context, and lately,extended to the antigenicity, immunity, hormone receptors, metabolism, growth rate and sensitivity to chemotherapy [1-4]. The heterogeneity of tumors,which may not be within the hormone receptor level in uniform, whether will affect the results of clinical testing. There will be little impact at present unknown. The accuracy of ER, PR and CerbB-2 testing in breast cancer is an important prerequisite for the endocrine therapy, and the key of good clinical treatment. In this study, it is mainly through comparisons of hormone receptor levels in the different harvesting methods of breast cancer tissue, and hollow needle aspiration specimens with paraffin section, to research the non-homogeneity of hormone receptor levels in breast cancer tissue, so as to provide a clinical reference work.Method: For 208 breast cancer patients in Breast Center of the Fourth Hospital of Hebei Medical University from July 2009 to February 2011, these tissues of breast cancer by the different harvesting Methods and Parts, were tested estrogen receptor,progesterone receptor and human epidermal growth factor receptor -2, with streptavidin peroxdase conjugated method,S-P.Group in detail:1.148 cases patients of breast cancer were randomly selected, which were confirmed by pathology with the preoperative core needle biopsy, and then taken the surgery. These tumor tissues of core needle biopsy and surgical resection have the paraffin embedding;2.Select the preoperative diagnosis, the preferred 60 patients of breast cancer surgery,excised the fresh samples of breast cancer tissue samples were harvested tumor center and four surrounding tumor tissue to the naked eye,placed in 10% formalin Immersion,embedded in paraffin. All specimens were pathologically confirmed again invasive cancer, using immunohistochemistry SP method, ER, PR and CerbB-2 protein expression of these specimens was detected, SPSS 13.0 version software package (SPSS Company, Chicago, Illinois, USA) on the data were analyzed to p <0.05 was statistically significant.Results:1 Compared results of immunol histochemistry on core needle biopsy aspiration and general paraffin.The test results of core needle biopsy specimens of 148 patients with breast cancer,114 cases ER positive, the positive rate was 77%; 102 cases PR positive, the positive rate was 69%; 73 cases CerbB-2 positive, the positive rate was 49%.The test results of general paraffin specimens of 148 patients with breast cancer, 111 cases ER positive, the positive rate was 75%; 95 cases PR positive, the positive rate was 64%; 57 cases CerbB-2 positive, the positive rate was 39 %. ER coincidence cases were 141 cases, the coincidence rate was 95.3%; PR coincidence cases were 131 cases, the coincidence rate was 88.5%; CerbB-2 coincidence cases were 108 cases, the coincidence rate was 73.0%. After matching tables from the Chi-square test, two different methods derived ER, PR, CerbB-2 was no significant difference in test results. (χ2=0.57,p=0.31;χ2=2.12,p=0.14;χ2=8.4,p=0.003) 2 Compared results of immunol histochemistry on cancer center position and peripheral position.The test results of cancer center position specimens of 60 patients with breast cancer, 41 cases ER positive, the positive rate was 68%; 34 cases PR positive, the positive rate was 57%; 27 cases CerbB-2 positive, the positive rate was 45%. The test results of cancer peripheral position specimens of 60 patients with breast cancer, 40 cases ER positive, the positive rate was 67%; 29 cases PR positive, positive rate was 48%; 23 cases CerbB-2 positive, the positive rate was 38%.ER coincidence cases were 55 cases, the coincidence rate was 91.7%; PR coincidence cases were 51 cases, the coincidence rate was 85.0%; CerbB-2 coincidence cases were 48 cases, the coincidence rate was 80%. After statistical analysis, two groups of ER, PR, CerbB-2 protein expression is no significant differences. (χ2=0,p=1.2;χ2=1.78,p=0.15;χ2=0.75,p=0.28)Conclusions:1 Compared with the gross specimen drawn in cancer center, the ER, PR, CerbB-2-positive rate is higher than the surrounding area, but it is no significant difference. It indicates that the present method can be derived for clinical pathology and provide a reliable immunol histochemic- -al indicators.2 Compared with the gross specimen drawn, core needle biopsy tissue Er and PR immunol histochemistry is slightly higher positive rate, but the two groups no significant difference. It is suggestion that the two methods measured ER and PR results can provide reliable indicators for clinical treatment.3 Compared with the gross specimen drawn, core needle biopsy tissue CerbB-2 immunol histochemistry is slightly higher positive rate, and the two groups have significant difference. It is suggestion that the core needle biopsy methods measured CerbB-2 results can provide more reliable indicators for clinical treatment.4 The different harvesting Methods and Parts of breast cancer tissue were consistent with the highest coincidence rate of ER, PR followed, and the maximum difference of CerbB-2.
