Objective: PSA,SA and A were used separately to intervene vulnerable carotid plaques,to retrospectively analyze the effect of the combination of atorvastatin, probucol and aspirin on the outcome of vulnerable carotid plaques,to explore the rule of outcome and the safe and effective treatment of vulnerable carotid plaques. Methods: A total of 199 patients with unstable carotid plaque were collected from June 2018 to September 2019,who were diagnosed with cerebrovascular disease at the Department of Neurology of China-Japan Union Hospital of Jilin University. They all received medical treatement and were reviewed for 3 months,6 months, and 12 months.Among them,52 patients(PSA group)were treated with combination of probucol, atorvastatin and aspirin,50 patients(SA group)were treated with atorvastatin combined with aspirin,55 patients(A group)were treated with aspirin and 42 patients(control group)were treated with aspirin irregularly or no medication. Before drug treatment and 3,6,12 months after treatment, all patients were examined as follows:(1)By measuring the peak systolic velocity(PSV)and end diastolic velocity(EDV)of the vulnerable carotid plaques with Color Doppler ultrasound, the stenosis of the lumen was evaluated;(2)The plaque area(PA)and Gray scale median(GSM)of each group were analyzed with Color Doppler ultrasound and Adobe Photoshop 7.0 software to assess plaque size and stability.(3)ECG and blood biochemical. The obtained data were analyzed and compared using SPSS22.0 software. Results: 1.After 12 months of treatment, the PA was significantly lower and the GMS was significantly higher than that before treatment in PSA and SA groups, the reduction of PA / the increase of GSM in PSA group was more significant than that in SA group; The PA and GSM were not significantly different from before treatment in A and control groups. The GSM compared with before treatment,PSA group had a statistically s ignificant increase after 3 months of treatment, and SA group had a statistically s ignificant increase after 6 months of treatment.2.After 12 months of treatment, the degree of luminal stenosis in PSA group was lower than that before treatment, but the difference was not statistically significant(P> 0.05);And there was no significant change in the degree of luminal stenosis at other times in other groups.3.There was no significant difference in TC,TG,LDL-C and HDL-C in serum of the four groups before treatment. After 12 months of treatment, the TC,TG,and LDL-C were significantly lower than those before treatment in PSA and SA groups, and the decrease of TC,TG and LDL-C in PSA group was more significant than those in SA group;TC,TG,LDL-C and HDL-C were not significantly different from those before treatment in A and control groups.4.During the 12-month treatment, the incidence of ischemic stroke was lower in PSA,SA,and A groups than in control group, and the incidence of ischemic stroke was lowest in PSA group.5.During treatment,no critical side effects were noted in four groups of patients. Conclusion: 1.The combination of atorvastatin, probucol and aspirin(PSA)and the combination of atorvastatin and aspirin(SA)have a significant effect on the carotid plaque area(PA)and gray scale median(GSM),suggesting that they can promote the transformation of vulnerable carotid plaque to stable plaque. The effect of PSA is more significant.2.The combination of atorvastatin, probucol and aspirin can promote vulnerable plaque to stable plaque earlier.3.During the 12-month treatment, the combination of atorvastatin, probucol and aspirin had the lowest incidence of stroke, and no critical s ide effects were noted. The combination of atorvastatin, probucol and aspirin is safe.
