Objective:To explore the therapeutic effect of combined infrared irradiation on the incision infection of gastrointestinal surgery on the basis of continuous negative pressure suction of the catheter, and provide a better treatment strategy for clinical gastrointestinal incision infection. Methods:Sixty patients with gastrointestinal incision infections who were included in the study from November 2017 to December 2019 in the second affiliated of Nanchang University were selected and divided into 4 groups according to different treatment methods,15 cases in each group.Routine treatment group:routine treatment according to the principles of incision infection treatment;tube negative pressure suction group:continuous negative pressure suction after conventional tube placement;infrared irradiation group:Infrared irradiation after routine processing.tube negative pressure suction combined with infrared group:continuous negative pressure suction combined with infrared radiation were used to treat incision infection.SPSS 21.0 statistical software was used to process the data. The results were compared by one-way analysis of variance(One-Way ANOVA)and the count data were compared by the?~2 test. The growth time of fresh granulation tissue, wound healing time of infected wounds(Sewing time after second-stage incision suture or complete filling time of granulation tissue),incision pain(based on NRS score of pain digital score),C-reactive protein value(CRP),number of positive neutrophils(N>75%),and the number of dressing changes(routine replacement every 2 days, external dressing soaked in time),among the four groups were compared.Results:(1)The general situation of the patients in the four groups before treatment:There were no statistical differences in the gender, age,BMI index,ASA score, albumin value, incision infection length and severity of incision infection in the four groups of patients before treatment(P>0.05).(2)Growth time and wound healing time of fresh granulation tissue of wounds:After treatment, the growth time and wound healing time of fresh granulation tissue of patients were significantly different among the four groups(P<0.05);Analysis and comparison among groups found that the continuous negative pressure suction combined with infrared irradiation was significantly better than the other three groups, and the difference was statistically significant(P<0.05).(3)Incision pain degree of infection:There was no significant difference in the score of incision pain before treatment in the four groups(P>0.05)and there were significant differences on the 1st,3rd and 7th days after treatment(P<0.05);The analysis and comparison among the groups showed that the continuous negative pressure suction combined with infrared irradiation treatment group could relieve the incision pain better than the other three groups. The difference was statistically significant(P<0.05).(4)C-reactive protein value:There was no significant difference in C-reactive protein value before treatment in the four groups(P>0.05),but the difference was significant on the 1st,3rd and 7th days after treatment(P<0.05);The inter-analysis comparison found that the value of C-reactive protein in the group of continuous negative pressure suction combined with infrared irradiation was significantly lower than that of the other three groups. The difference was statistically significant(P<0.05).(5)Number of positive neutrophils(N>75%):There was no statistically significant difference in the number of positive neutrophils among the four groups before treatment(P>0.05),but 1st,3rd and 7th days after treatment There are differences among the four groups(P<0.05);Comparative analysis among the groups shows that the number of positive cases of neutrophils in the continuous negative pressure suction combined with infrared irradiation group is less than that in the conventional treatment group and the infrared irradiation group.Significance in learning(P<0.05),but there was no significant difference on the 1st,7th day after treatment compared with the continuous negative pressure suction group(P>0.05).(6)Number of dressing changes:After a period of treatment, there were significant differences in the number of dressing changes among the four groups(P<0.05);Analysis and comparison among four groups found that continuous negative pressure suction after intubation combined with infrared irradiation can significantly reduce the number of dressing changes. The difference was statistically significant(P<0.05).Conclusion:For the treatment of gastrointestinal postoperative incision infections, continuous negative pressure suction combined with infrared irradiation can significantly promote the growth of fresh granulation tissue, promote incision healing, relieve incision pain, reduce inflammatory indicators and reduce the number of dressing changes compared with the conventional treatment of infrared irradiation, continuous negative pressure suction and routine treatment group. The combined use of the two is safe and effective, and it is worthy of clinical promotion.
