Research Background:According to the report on cardiovascular diseases in China 2018,the mortality rate of cardiovascular diseases is the highest,higher than that of tumor and other diseases,accounting for more than 40% of the death rate of residents.Generally speaking,the prevalence and mortality rate of cardiovascular diseases in China are still on the rise,and the incidence of coronary heart disease has been on a significant rise in recent years [1].According to the data released by the world health organization in 2006,there are 16.7 million cardiovascular deaths every year,accounting for 29.2% of the total deaths,including 7.2 million coronary heart disease deaths [2].The high prevalence and mortality of coronary heart disease bring new challenges to the early and effective prevention and treatment of cardiovascular diseases.Research purposes:Calculated in the first affiliated hospital of nanchang university heart line with coronary angiography in patients with PNI score(including serum albumin levels,lymphocyte count)and coronary artery lesion severity,comparison of patients with coronary heart disease(CHD)with normal coronary angiography PNI score(including serum albumin levels,lymphocyte count),and the PNI score(including serum albumin levels,lymphocyte count)study the correlation between severity of coronary lesions and discussed in this paper.Research objects and methods:The first coronary angiography results of 512 patients admitted to the department of cardiology of the first affiliated hospital of nanchang university on September 1,2019 were retrospectively analyzed.According to the number of coronary artery lesions shown by angiography,the patients were divided into three groups: normal coronary artery group,single-vessel lesion group,double-vessel lesion group and three-vessel lesion group.The international Gensini scoring standard was used to score the angiographic results of the patients.The patients were divided into three groups: high risk group(Gensini score ?64 points),medium risk group(41points ?Gensini score < 64 points)and low risk group(Gensini score < 41 points).General data(sex,age,smoking history,drinking history,hypertension history,diabetes history)and laborature-related test results(such as serum albumin,lymphocyte count,etc.)of patients in the above six groups at the time of admission were collected,and the prognostic nutritional index(PNI)score was calculated.Using the relevant statistical methods the clinical data of the above groups separately carries on the analysis and comparison between groups,there are significant differences on the variables of the statistical analysis by orderly logistic regression,and then select independent risk factors for coronary heart disease coronary artery lesion severity,the independent risk factors for ROC curve analysis in order to understand its predictive value,through the Spearman correlation analysis of the nutrition index of prognosis(PNI)and correlation between the severity of coronary artery lesions.SPSS17.0software was used for the above statistical analysis.Result:(1)how many is divided into three groups of patients with coronary artery lesion count between age,sex,smoking history,history of hypertension,diabetes,white blood cells,LY,fibrinogen,D-dimer,albumin,uric acid,LDH and CK-MB,total cholesterol,LDL,HDL,Gensini score and PNI had significant difference(P <0.05),are lesion group was higher than normal coronary arteries.However,there was no significant difference among the three groups in alcohol consumption history,RBC,PLT,Hb,INR and triglyceride(P>0.05).(2)Orderly Logistic regression analysis was conducted with the number of patients with coronary artery disease as the dependent variable and various risk factors such as age,gender,lymphocyte and albumin as independent variables.The results showed that age,albumin and PNI entered the equation as the risk factors for multiple coronary artery disease(P <0.05).(3)Gender,smoking history,diabetes history,WBC,FIB,albumin,uric acid,LDH,ck-mb,total cholesterol,HDL,LDL,and number of lesions among the three groups were statistically different(P < 0.05).However,there was no significant difference in age,history of alcohol consumption,history of hypertension,RBC,Hb,PLT,LY,INR,d-dimer and triglyceride among the groups(P > 0.05).(4)The Gensini score of patients with coronary lesions was taken as the dependent variable,and the age,gender,lymphocyte,albumin and other risk factors were taken as independent variables to conduct orderly Logistic regression analysis.The results showed that albumin,smoking history and PNI entered the equation and were the risk factors of the high-risk group of coronary lesions(P <0.05).(5)ROC curve: the area under the ROC curve of gender was 0.604,the area under the ROC curve of smoking history was 0.392,the area under the ROC curve of lymphocytes was 0.559,the area under the ROC curve of albumin was 0.786,and the area under the ROC curve of PNI was 0.834.(6)Spearman correlation analysis was used to analyze the correlation between PNI and coronary artery severity(gensini score),and the results showed that there was a negative correlation between PNI and gensini score(r=-0.539,P < 0.01).Conclusion:The nutritional status of patients undergoing coronary angiography at admission(assessed by prognostic nutritional index PNI)has a moderate correlation with the severity of coronary artery lesions.PNI score can be used as one of the effective predictors of the severity of coronary artery lesions,and can be used to assist preoperative evaluation of the severity of coronary artery lesions in patients with coronary angiography.
