A Meta-analysis Of The Clinical Efficacy Of Traditional Chinese And Western Medicine In The Treatment Of HP-positive PU And The Law Of Application Of Traditional Chinese Medicine

BACKGROUND:Peptic ulcer(PU)is a common clinical disease.About 90% of PU is related to Helicobacter pylori infection(HP).Due to the problems of antibiotic resistance and increased adverse reactions of proton pump inhibitors,the effectiveness of drug programs to eradicate HP has generally decreased,the problem of HP recurrence is very serious,and the difficulty of HP-positive PU treatment has escalated.In the past ten years,Chinese Scholar continuously explored the use of different traditional Chinese medicine decoctions in combination with omeprazole,amoxicillin and clarithromycin for the treatment of HP-positive PU.Is this an effective method to completely and effectively eradicate Hp and cure PU? Further demonstration and discussion.Objective: To systematically evaluate the clinical efficacy of traditional Chinese medicine decoction in combination with western medicine triple therapy VS triple therapy in the treatment of HP-positive PU.Methods: Develop a literature search strategy and determine the inclusion and exclusion criteria.Search Chinese medicine decoctions combined with omeprazole,amoxicillin,and clarithromycin in five websites of China CNKI,Wanfang,Weipu,web of science,and pubmed.The literatures of the positive PUs were collected and sorted by Excel,Review Manager 5.3,and stata software,and plotted and tabulated.The analysis results were concluded.Results: A total of 31 clinical studies were included,and a total of 3208 HP-positive PU patients.The meta-analysis results showed that the traditional Chinese medicine decoction combined with omeprazole,amoxicillin and clarithromycin had clinical effectiveness(combined effect size OR = 3.68,Confidence interval 95% CI: 2.91 to 4.67),ulcer healing rate(OR = 2.27,95% CI: 1.95 to 2.64),HP eradication rate(RR = 1.27,95% CI: 1.22 to 1.32),disease recurrence rate(RR = 0.33,95% CI: 0.25 to 0.43),adverse reactions(RR = 0.53,95% CI: 0.37 to 0.74),and symptoms of abdominal pain(SMD =-2.01,95% CI:-2.46 to-1.56),Acid(SMD =-1.42,95% CI:-1.71 ~-1.14)were all better than simple western medicine triple therapy,and the results were statistically significant;the included Chinese medicine prescriptions included Yiqi drugs,Activating blood.hemostatic drugs,heat-clearing drugs,and phlegm-reducing drugs are mainly combined with the prescription.The single-drug selection is mainly based on kuganxin and warm cold,and the main meridian is spleen and stomach,and liver meridian.Commonly used Chinese herbal medicines are: baishao + Licorice,dangshen + Licorice and so on.Conclusion: Traditional Chinese medicine decoction combined with western medicine triplet can not only effectively increase the cure rate and HP eradication rate of HP-positive PU,reduce the chance of HP recurrence,but also reduce the occurrence of adverse reactions caused by the same medication and alleviate common clinical symptoms.Patients with newly treated and HP-positive ulcers should actively intervene with symptomatic Chinese medicine decoctions.
