Objective:To explore the difference of sleep quality and cognitive function between patients with depression and healthy subjects;to explore the relationship between symptoms, sleep quality and cognitive function of patients;to explore the effect of five elements music therapy on sleep quality and cognitive function of patients with depression.Method:In accord with the international health problems and the related statistical classification of diseases(ICD-10)depression diagnostic criteria, this study included a total of 80 patients with depression. They were randomly divided into intervention group(n=40)and the control group(n=40).Seventy-four healthy controls were included in the same period. On the basis of routine drug therapy and sleep health education, the intervention group received five elements of music therapy, and the control group had no special intervention for 2 weeks. Before and after the intervention, the cognitive function was evaluated by the Repeatable Battery for the Assessment of Neuropsychological Status(RBANS)and Stroop Color-Word Test;sleep respiratory monitoring was used to evaluate sleep quality;Hamilton Depression scale and Hamilton Anxiety scale were used to evaluate symptoms.Result:1.At baseline, patients(intervention group control group)and healthy control group had significant differences in the levels of sleep monitoring indicators and dimensions of cognitive function in addition to apnea hypopnea index(AHI),the visuospatial/constructional score and the language score(P<0.05);2.There was a significant positive correlation between depression symptom score and anxiety symptom score(P<0.01),while there were significant negative correlations with the immediate memory score, attention score, delayed memory score and total score of RBANS(P<0.05).The anxiety symptom score was significantly negatively correlated with the immediate memory score(P<0.05).A significant negative correlation was found between the scores of depressive symptoms and the proportion of sleep efficiency and eye movement period(P<0.01).And a significant positive correlation was found between the scores of depression symptoms and the proportion of awake period(P<0.01).The anxiety symptom score was significantly negatively correlated with sleep efficiency(P<0.01)and positively correlated with the proportion of awake period(P<0.01).There were significant positive correlations between sleep efficiency and the immediate memory score, the attention score, the delayed memory score, the total score of RBANS,the words score, the color score and the color-word score(P<0.05).The proportion of awake period was negatively correlated with the immediate memory score, the attention score, the delayed memory score, the total score of RBANS,the words score, the color score and the color-word score(P<0.05).Eye movement period proportion was significantly positive correlation with the words score and the color score(P<0.05).The proportion of shallow sleep period was significantly positively correlated with the immediate memory score, the delayed memory score, and the total score of RBANS(P<0.05),while it was significantly negatively correlated with the word score(P<0.05).A significant positive correlation was found between the proportion of deep sleep period and the immediate memory score, the attention score, the word score, the color score, and the color word score(P<0.05).3.The differences of intervention group between before and after intervention were statistically significant in sleep efficiency, the proportion of awake period, the immediate memory score, the attention score, the delayed memory score, the total score of RBANS,the color words, the depressive symptoms score and the anxiety symptoms scores(P<0.05).4.At the post-test,the sleep efficiency and the delayed memory score in the intervention group were higher than those in the control group, while the proportion of awake period and the anxiety symptoms score were lower than those in the control group, and the differences were statistically significant(P<0.05).Conclusion:1.Compared with healthy subjects, depressed patients had lower sleep quality and cognitive function.2.For the patients, there were significant negative associations between depressive symptoms and sleep quality and cognitive performances,so were anxiety symptoms. There is a positive correlation between sleep quality and cognitive function in depressed patients.3.Five-elements music therapy can effectively improve sleep quality and cognitive function in patients with depression.4.Five-elements music therapy can effectively relieve anxiety symptoms of depressed patients.
