Observation On The Clinical Efficacy Of External Application Of Huoxue Huayu Powder Combined With Core Decompression And Bone Grafting In The Treatment Of Early And Mid-term Femoral Head Necrosis

Objective: This article retrospectively studied the clinical data of 60 patients with femoral head necrosis in the early and mid-term were treated with using self-made Huo xue Hua yu Powder topical combined with pulp core decompression,allograft bone implant,allograft fibula support(treatment group)and simple pulp core decompression,allograft bone implant,allograft fibula support(control group),from October 2014 to October 2015.The therapeutic effects of the two treatment options were compared and confirmed the feasibility of self-made Huo xue Hua yu Powder topical combined with pulp core decompression,allograft bone implant,allograft fibula support for the treatment of femoral head necrosis in the early and mid-term,and provided a new treatment plan for the combination of TCM and Western medicine to protect the hip joint.Methods:Thirty patients with osteonecrosis of the femoral head treated with using Huo xue Hua yu Powder topical combined with pulp core decompression,allograft bone implant,allograft fibula support were collected.And thirty patients that using simple pulp core decompression,allograft bone implant,allograft fibula support operation for protecting the hip joint were collected.The postoperative erly period,all patients were not weightd and local rehabilitation therapy.Reularly followed up postoperation,average of 26.37±2.72 months.The Harris score,pain(visual analogue scale VAS score),and hip functional activity change before and after the treatment were compared between the two protect hip treatment methods;the finaly outpatient visit was from the progress rate of femoral head necrosis indicated by ordinary X-ray images was expressed as excellent,good,fair,and poor,the effective percentage between the two groups was calculated.Results:(1)Long-term follow-ups were obtained in both groups of patients during the outpatient period.During this period,the patient did not perform any relevant treatment other than the study.At the same time,long-term heavy drinkers followed doctor's advice and abstention from alcohol.The preoperative Harris score was 72.17±8.68 points in the treatment group and 72.53±5.54 points in the control group.There was no statistical difference and they were comparable(P>0.05).The last follow-up Harris score was 88.30±5.48 points in the treatment group.There was significant clinical difference before operation(P<0.05).The last follow-up of Harris score was 82.63±4.42 points after operation in the control group.There was also significant clinical difference before operation(P<0.05);Harris score was compared in the last follow-up between the two groups.The score of the group was higher than that of the control group and there was a statistical difference(P<0.05).(2)Before treatment,the VAS score of the treatment group was 3.57±0.94,and the VAS score of the last follow-up treatment group was 1.37±0.56.The VAS score of the control group before treatment was 3.50±0.51,and the VAS score of the last follow-up control group was 1.80.± 0.96;the two groups were significantly lower than the control group in the treatment group,with statistical significance(P <0.05).(3)Before treatment,hip function was 261.03±17.04 in the treatment group,281.60±25.64 in the last follow-up group,and 246.10±46.22 in the control group before the treatment.The hip function activity in the group was 265.07±32.66.Compared with the control group,the motion of the two groups was significantly higher than that of the control group(P<0.05).(4)The normal radiographic evaluation of the last follow-up showed that the effective rate was 90.0% in the treatment group and 76.7% in the control group.The treatment efficiency was higher in the treatment group than in the control group.Conclusion:(1)Huo xue hua yu Powder combined with c pulp core decompression,allograft bone implant,allograft fibula support is simpler than pulp core decompression,allograft bone implant,allograft fibula support for early and mid-term stocks.Patients with bone necrosis have significant clinical efficacy in improving Harris hip score,pain score,and joint function activity.(2)Huo xue hua yu Powder has the effect of promoting blood circulation,removing blood stasis,and relieving pain.(3)Huo xue Hua yu Powder external application combined with pulp core decompression,allograft bone implant,allograft fibula support hip-preserving treatment can delay the early and mid-term necrosis of femoral head collapse,and it is worthy of further study in clinic.
