This paper is divided into two parts.The first part is the theoretical analysis.the specific research is as follows:Objective:To study the relationship between spleen and brain,find out its correlation,explore the theoretical,scientific and clinical application of encephalopathy from spleen,and provide theory for the clinical application of spleen and qi method in the treatment of cerebral ischemia-reperfusion Based on,it also provides new ideas and new methods for clinical prevention and treatment of cerebrovascular diseases.Methods:Through the study of the understanding of the spleen and brain in the "Nei Jing" and the study of the spleen and brain by the later doctors,the connection between the spleen and the brain is explored,and on this basis,the idea of "spleen-brain-related" is put forward,and further "encephalopathy from the spleen" The new concept of "governing".Results:The spleen is closely related to the brain's physiology,and the spleen and brain are also closely related.Conclusion:From the "Nei Jing" and the later generation doctors' understanding of "spleen and brain related" and "encephalopathy from the spleen",it can be inferred that the spleen and qi method has a powerful dialectical,unique and feasible treatment for stroke.Treating encephalopathy from the spleen also shows new ideas and methods for the treatment of many modern encephalopathy.It has a far-reaching impact on effectively improving the cure rate of encephalopathy and improving the quality of life of patients.The second part is the experimental research,the specific research is as follows:Objective:To observe the expression of neuronal extracellular matrix Fibulin-5,P-Akt,P-BAD and apoptotic factors Bcl-2 and caspase-3 after cerebralischemia-reperfusion(I/R)injury in rats with Jianpi Yiqi Decoction.The effect of the situation was to explore the intervention mechanism of Jianpi Yiqi Decoction on neuronal apoptosis after cerebral ischemia-reperfusion injury in rats.Methods:120 male SD rats were randomly divided into Sham-operated group(n=24)and model group(n=96).The MCAO model was established by the method of thread embolization.After the establishment of the model,the rats were divided into five groups: model group,edaravone group,high dose group and small dose group..The rats were sacrificed in batches after 3 days and 7 days.The neurological deficits were scored before sacrifice.The histopathological observation was performed by HE staining.Immunohistochemistry was used to detect the expression of neuronal extracellular matrix Fibulin-5,P-Akt,P-BAD and apoptotic factors Bcl-2 and caspase-3.Apoptotic nerve cells were detected by Tunel staining.Results:3 days group: Compared with the sham operation group,the neurological function scores of the model group were significantly increased(P<0.01),the neuronal apoptosis index(AI)was significantly increased,the Fibulin-5 expression was decreased,and the caspase-3 expression was significantly increased.The expressions of P-Akt,P-BAD and Bcl-2 were significantly decreased,and the differences were statistically significant(P<0.05 or P<0.01).Compared with the model group,the positive drug group and the Jianpi Yiqi Decoction were larger in each dose group.There was no significant decrease in the neurological function score(P>0.05)and no decrease in the neuronal apoptosis index(AI).The expression of Fibulin-5,P-Akt and Bcl-2 positive cells in the brain tissue of each group of edaravone and Jianpi Yiqi Decoction were not significantly increased,and the expression of caspase-3 positive cells was not significantly decreased.The difference was not statistically significant(P >0.05).Compared with the model group,the P-BAD in the brain tissue of each group of edaravone and Jianpi Yiqi Decoction was significantly enhanced,and the difference was statistically significant(P<0.05).7 days group: Compared with the sham operation group,the neurological scores of the model group were significantly increased(P<0.01),the neuronal apoptosis index(AI)was significantly increased,the Fibulin-5 expression wasdecreased,and the caspase-3 expression was significantly increased.The expressions of P-Akt,P-BAD and Bcl-2 were significantly decreased,and the differences were statistically significant(P<0.05 or P<0.01).Compared with the model group,the positive drug group and the Jianpi Yiqi Decoction were larger in each dose group.The neurological function score was decreased(P<0.05),and the neuronal apoptosis index(AI)was significantly decreased.The expression of Fibulin-5 was increased,the expression of caspase-3 was significantly decreased,and the expression of P-Akt,P-BAD and Bcl-2 was significantly increased(P<0.01 or P<0.05).Conclusion:Jianpi Yiqi Decoction can improve the neurological deficit of(I/R)rats,promote the recovery of nerve function and reduce nerve cell damage.Jianpi Yiqi Decoction up-regulates the expression of extracellular matrix Fibulin-5,decreases the expression of caspase-3 protein,increases the expression of P-Akt,P-BAD and Bcl-2 proteins,and decreases the apoptotic index AI,suggesting that Jianpi Yiqi may By stabilizing the extracellular matrix of neurons,it inhibits cell apoptosis and protects against cerebral ischemic injury.The protective and therapeutic effects of Jianpi Yiqi Decoction on cerebral ischemia showed obvious time-effect relationship between the experimental results and the administration time.There was no statistical significance in the 3-day group,but significantly improved in the 7-day group.Therefore,the time for Jianpi Yiqi Decoction should not be too short when used for treatment.It is recommended to be no less than 7 days.
